Vitamin B3, also known as nicotinamide or niacin (note: nicotinamide is a form of niacin, but the lipid-lowering effects are primarily associated with the niacin form), has substantial scientific evidence supporting its use in lowering triglyceride levels. Niacin has been used since the 1950s as a lipid-modifying agent. Multiple clinical trials have demonstrated that pharmacological doses of niacin can significantly reduce triglyceride levels (by 20-50%), as well as lower total cholesterol and LDL cholesterol, while increasing HDL cholesterol. The mechanism is thought to involve inhibition of hepatic diacylglycerol acyltransferase-2, which decreases the synthesis of very low-density lipoprotein (VLDL), a precursor to triglycerides in the blood.

However, it is important to differentiate between nicotinic acid (niacin) and nicotinamide. While both are forms of vitamin B3, only nicotinic acid (not nicotinamide) has been shown to have lipid-lowering effects. Nicotinamide does not improve lipid profiles at standard doses. Niacin therapy can be effective, but is associated with side effects such as flushing, liver toxicity, and glucose intolerance, limiting its widespread use. Major cardiovascular outcome trials (e.g., AIM-HIGH and HPS2-THRIVE) have not demonstrated significant additional benefit of niacin when added to statin therapy, but its triglyceride-lowering effect is well-established.

In summary, there is strong scientific evidence for the use of niacin (vitamin B3, but not nicotinamide) in lowering triglycerides, though its clinical use has declined due to side effects and limited outcome benefits when used with statins.
