Andrographolide is a diterpenoid lactone isolated from Andrographis paniculata, a medicinal plant used traditionally in Asia for various ailments, including infections. In the context of Tuberculosis (TB), there is emerging but limited scientific evidence supporting its use. Preclinical studies have demonstrated that andrographolide possesses immunomodulatory and anti-inflammatory properties, as well as some direct antimycobacterial activity [1]. For example, in vitro experiments have shown that andrographolide can inhibit the growth of Mycobacterium tuberculosis, the causative agent of TB, and modulate immune responses by enhancing macrophage activity [2]. Animal studies have suggested that andrographolide may reduce bacterial load and pulmonary inflammation in TB-infected mice.

However, robust clinical trials in humans are lacking. No large-scale, peer-reviewed clinical studies have established its efficacy or safety as a stand-alone or adjunct therapy for TB. Most evidence remains at the preclinical or early-stage research level. Thus, while the scientific rationale exists and preliminary data are promising, the overall quality and quantity of evidence are limited, justifying a moderate evidence rating (2/5).
