Tylophorine is an alkaloid derived from the plant Tylophora indica, which has a long history of use in traditional Ayurvedic and folk medicine in India. Historically, the leaves of this plant have been used for various respiratory ailments, including asthma, bronchitis, and, notably, tuberculosis (TB). The traditional use of Tylophora indica for TB is likely based on its broad application for chronic coughs and respiratory complaints, which are prominent symptoms of TB. However, while several in vitro and animal studies have demonstrated immunomodulatory, anti-inflammatory, and some antimicrobial properties of tylophorine and related compounds, robust clinical evidence in humans for its efficacy against Mycobacterium tuberculosis is lacking. No large-scale, randomized controlled trials have confirmed its safety or effectiveness for treating TB, and it is not recognized in modern clinical guidelines as a treatment for this condition. Thus, while the use of tylophorine for TB is justified by traditional practices, scientific validation is limited and does not currently support its use as a primary or adjunct therapy for tuberculosis.
