Lactobacillus pentosus is a species of lactic acid bacteria found in fermented foods and as part of the natural microbiota of the human vagina. Its use to support or treat urinary tract infections (UTIs) is based on scientific interest in the role of probiotic lactobacilli in restoring and maintaining healthy urogenital flora. Several strains of Lactobacillus, especially L. crispatus, L. rhamnosus, and L. reuteri, have been studied more extensively for UTI prevention. L. pentosus has been less commonly investigated, but some in vitro research suggests it can adhere to uroepithelial cells and inhibit the growth of uropathogens such as Escherichia coli. A few small clinical studies and case reports have included L. pentosus as part of multi-strain probiotic formulations used for vaginal or urinary tract health, but high-quality randomized controlled trials (RCTs) specifically focused on L. pentosus for UTI prevention or treatment in humans are lacking. Therefore, while there is some scientific rationale and preliminary in vitro evidence supporting its potential role, direct clinical evidence is limited and less robust than for other lactobacilli. Overall, the evidence for using L. pentosus in UTI support is emerging but not yet strong, justifying a moderate-low score.
