D-mannose is a naturally occurring sugar that has gained attention for its potential role in supporting urinary tract health, particularly in the prevention and management of uncomplicated urinary tract infections (UTIs). Scientific evidence, including several randomized controlled trials and meta-analyses, suggests that D-mannose may reduce the risk of recurrent UTIs, especially in women. The mechanism is thought to involve D-mannose binding to Escherichia coli (E. coli) bacteria, the most common cause of UTIs. This binding prevents the bacteria from adhering to the lining of the urinary tract, allowing them to be flushed out during urination.
A 2014 randomized clinical trial (Kranjčec et al.) found that D-mannose powder was as effective as the antibiotic nitrofurantoin in preventing recurrent UTIs over six months, with fewer side effects. A 2022 meta-analysis (Lenger et al.) concluded that D-mannose significantly reduced the risk of recurrent UTIs compared to placebo and may be comparable to antibiotics for prevention, though the quality and size of studies are still limited. D-mannose is generally well-tolerated, with gastrointestinal upset being the most commonly reported side effect.
While the data are promising, larger, high-quality studies are still needed to confirm its efficacy and optimal dosing. Nonetheless, current research provides a moderate level of scientific support for the use of D-mannose in managing and preventing uncomplicated UTIs.
