Methenamine is scientifically validated for use in the prevention of recurrent urinary tract infections (UTIs), though it is not typically used to treat acute infections. Methenamine works as a urinary antiseptic: it is hydrolyzed to formaldehyde in acidic urine (pH <6), and formaldehyde acts as a nonspecific bactericidal agent. Its efficacy has been demonstrated primarily in prophylaxis—reducing the frequency of recurrent UTIs—rather than in the treatment of active or complicated infections.
Multiple randomized controlled trials and systematic reviews have shown that methenamine hippurate significantly reduces the risk of recurrent UTIs when compared to placebo, especially in women with frequent recurrences.
