Ovate Buchu (Agathosma crenulata) has a long history of traditional use in South African herbal medicine, particularly among the Khoisan and other indigenous peoples, for urinary tract conditions, including burning or painful urination (dysuria). The aromatic leaves of buchu have been prepared as teas or tinctures and employed as diuretics, as well as for their purported antiseptic and anti-inflammatory properties in the urinary system. In the 19th and early 20th centuries, buchu was also adopted into European and North American herbal pharmacopeias to support urinary health and was included in several official compendia. However, despite its long-standing traditional use, the scientific evidence supporting buchu's effectiveness for treating burning or painful urination is limited. Contemporary studies on Agathosma species are sparse, with most research focusing on its chemical constituents (such as diosphenol and various flavonoids) rather than robust clinical trials in humans. Some in vitro studies suggest mild antimicrobial activity, but there is insufficient clinical data to confirm efficacy in urinary tract infections or dysuria. As a result, the use of ovate buchu for burning or painful urination is primarily justified by traditional use, with only weak supportive evidence from modern science.
