D-mannose is a naturally occurring simple sugar that has gained popularity primarily for its role in supporting urinary tract health, particularly in the context of urinary tract infections (UTIs). The scientific rationale behind D-mannose use is based on its ability to inhibit the adhesion of certain bacteria, especially Escherichia coli (E. coli), to the lining of the urinary tract. Most clinical studies on D-mannose focus on its effectiveness in reducing the recurrence of UTIs, not specifically on treating or reducing the symptom of frequent urination in the absence of infection.

There is some scientific evidence that D-mannose can reduce UTI recurrence, which may indirectly reduce frequent urination if that symptom is due to infection. A meta-analysis (2022) and several randomized controlled trials have shown that D-mannose supplementation can be as effective as some antibiotics in preventing recurrent UTIs in women. Symptom relief, including frequency of urination, occurs as a result of infection resolution rather than a direct effect on urinary frequency. There is little evidence to support the use of D-mannose for frequent urination unrelated to infection, such as that caused by overactive bladder, diabetes, or other non-infectious causes.

In summary, while there is scientific support for D-mannose in preventing recurrent UTIs—which may secondarily address frequent urination due to infection—there is limited evidence for its direct use in treating frequent urination as a standalone symptom.
