Pygeum (Prunus africana) is commonly used to support urinary health, particularly in men experiencing frequent urination due to benign prostatic hyperplasia (BPH). Its use is supported by a moderate body of scientific evidence. Several clinical trials and meta-analyses have found that pygeum extract can reduce urinary symptoms associated with BPH, such as nocturia (frequent nighttime urination), hesitancy, and incomplete bladder emptying. The proposed mechanisms include anti-inflammatory effects and inhibition of 5-alpha-reductase, an enzyme involved in prostate enlargement.

A Cochrane review (Wilt et al., 2002) analyzed 18 randomized controlled trials involving over 1,500 men and found that pygeum improved overall urinary symptoms and flow measures compared to placebo. However, the studies reviewed were generally small and of varying quality, leading to moderate confidence in the results. Recent research continues to support pygeum’s potential benefit, although more high-quality, large-scale trials are needed for definitive conclusions.

Historically, pygeum has been used in African traditional medicine for urinary and bladder disorders, providing a traditional basis for its application. However, its use in modern supplements is primarily grounded in scientific investigation.
