Flavan-3-ols, a subclass of flavonoids found in foods such as cocoa, tea, apples, and grapes, have been investigated for their vascular benefits, including potential effects on chronic venous insufficiency (CVI) and varicose veins. Several randomized controlled trials and meta-analyses suggest that flavonoid compounds—particularly those derived from citrus (diosmin, hesperidin) and grape seed extract (rich in flavan-3-ols/proanthocyanidins)—can improve symptoms related to varicose veins such as leg swelling, pain, and heaviness. The European Society for Vascular Surgery guidelines (2022) mention the use of certain flavonoid-rich preparations as venoactive drugs, citing moderate-quality evidence for symptom relief in CVI.

The proposed mechanisms include strengthening capillary resistance, reducing capillary permeability, and exerting antioxidant and anti-inflammatory effects on the venous wall. However, while the evidence supports symptomatic improvement, there is limited data indicating that flavan-3-ols reverse or cure underlying venous pathology.

It should be noted that most clinical studies pool various flavonoids together, and results often pertain to specific extracts rather than isolated flavan-3-ols. Still, grape seed proanthocyanidin extract, rich in flavan-3-ols, has shown efficacy in several small- to moderate-sized trials.

Overall, scientific evidence supports the use of flavan-3-ol-rich extracts as adjuncts for symptomatic relief in varicose veins, but more robust, long-term studies are needed to clarify their effects on disease progression.
