Proanthocyanidins, particularly those derived from grape seed extract (Vitis vinifera), have been studied for their potential benefits in supporting vascular health, including the treatment of varicose veins. These compounds are a class of flavonoids with antioxidant properties, believed to strengthen blood vessel walls and improve microcirculation. Several randomized controlled trials and meta-analyses have investigated proanthocyanidins in chronic venous insufficiency (CVI), which often presents with symptoms similar to varicose veins such as swelling, pain, and heaviness in the legs.

A 2016 meta-analysis published in Phytotherapy Research concluded that proanthocyanidin-rich extracts led to significant improvement in leg edema and other symptoms of CVI compared to placebo. Mechanistic studies suggest these effects may be due to increased capillary resistance and reduced vascular permeability. The European Medicines Agency recognizes extracts containing oligomeric proanthocyanidins as traditional herbal medicinal products for the relief of minor venous circulatory disturbances.

However, while the evidence is promising and some guidelines mention these extracts as an adjuvant therapy, the number of high-quality long-term studies specifically on varicose veins (not just CVI in general) is limited. Overall, scientific evidence supports the use of proanthocyanidins for symptoms associated with venous insufficiency, but further large-scale trials are needed to confirm their direct benefit in varicose veins.
