Red Horse Chestnut (Aesculus × carnea) is a hybrid between the common horse chestnut (Aesculus hippocastanum) and the red buckeye (Aesculus pavia). The use of horse chestnut seed extract for the treatment of varicose veins and chronic venous insufficiency (CVI) has scientific support, primarily for Aesculus hippocastanum. Several clinical trials and meta-analyses indicate that horse chestnut seed extract standardized to aescin can reduce leg swelling, pain, and symptoms associated with varicose veins and CVI. The German Commission E has approved horse chestnut seed extract for these indications. However, most of this scientific evidence specifically refers to Aesculus hippocastanum, not the red horse chestnut hybrid (Aesculus × carnea). The active constituent, aescin, is present in both, but the concentration and safety profile in Aesculus × carnea are less well-studied. Thus, scientific validation exists for the general use of horse chestnut seed extract for varicose veins, but direct evidence for red horse chestnut is limited. The rating reflects that while the broader genus is supported, the hybrid’s specific efficacy is inferred rather than directly established in clinical research.
