Rutosides (also known as rutin or rutoside) are a group of bioflavonoids that have been used in the management of chronic venous insufficiency (CVI) and varicose veins. The proposed mechanism includes strengthening capillaries, reducing capillary permeability, and exerting anti-inflammatory effects. Several randomized, double-blind, placebo-controlled trials, particularly with oxerutins (a mixture of hydroxyethylrutosides), have demonstrated symptom improvement in patients with CVI, including leg swelling, pain, and heaviness—symptoms also present in varicose vein pathology. Systematic reviews and meta-analyses (e.g., Cochrane reviews) have found modest but significant effects on edema reduction and symptom relief compared to placebo, though the clinical relevance of these effects is debated due to variability in study quality and size. European guidelines for chronic venous disease sometimes mention rutosides as a therapeutic option, especially when other treatments are unsuitable. However, evidence is not strong enough for universal recommendation, and most studies are short-term. No major safety concerns have been reported, but long-term efficacy and safety data are lacking. In summary, rutosides have some scientific validation for improving symptoms associated with varicose veins and chronic venous insufficiency, but the strength of evidence is moderate and not definitive.
