Durabolin (nandrolone phenylpropionate) is an anabolic-androgenic steroid that has been used in medical practice to treat various catabolic and wasting conditions. The scientific rationale for its use lies in its anabolic properties, which promote protein synthesis and muscle growth while inhibiting protein breakdown. Clinical studies and medical reports from the 1960s through the 1990s document the use of nandrolone and related compounds in the treatment of muscle wasting associated with chronic illnesses such as HIV/AIDS, cancer, and severe trauma.

Nandrolone has been shown in controlled trials to increase lean body mass, improve nitrogen balance, and enhance muscle strength in patients experiencing wasting. For example, studies in HIV-positive patients with cachexia demonstrated improvements in muscle mass and sometimes quality of life when treated with nandrolone decanoate (a related, longer-acting ester of nandrolone). The evidence shows a clear anabolic benefit, although concerns about possible androgenic and cardiovascular side effects have limited its long-term use.

Despite its clinical efficacy, nandrolone is rarely used today for this indication due to the development of alternative therapies with fewer side effects, such as appetite stimulants and newer anabolic agents. Its use is now limited by strict regulation due to potential for abuse. Nevertheless, the scientific evidence supporting nandrolone’s efficacy in the treatment of wasting syndromes is robust, meriting a high rating for evidence.
