Ibutamoren (also known as MK-677) is a non-peptide, orally active growth hormone secretagogue. It functions by mimicking the action of ghrelin and stimulating the growth hormone (GH) receptor, leading to increased secretion of GH and insulin-like growth factor 1 (IGF-1). Several clinical studies have evaluated Ibutamoren's effects on body composition, particularly in populations susceptible to muscle wasting or catabolic states, such as the elderly or individuals with GH deficiency.
Evidence indicates that Ibutamoren can increase lean body mass primarily through enhanced muscle growth and water retention. For example, a randomized, double-blind, placebo-controlled trial in healthy older adults found that daily oral administration of Ibutamoren for 2 months significantly increased fat-free mass compared to placebo. Other studies have shown improved nitrogen balance and prevention of muscle wasting in patients with catabolic conditions.
While Ibutamoren reliably increases appetite (due to its ghrelin-mimetic activity), this effect can also contribute to weight gain. However, data on its effects on fat mass are mixed, and most gains are attributed to lean tissue and water. Importantly, its use outside of clinical trials is not approved by regulatory agencies, and long-term safety data are limited. Overall, there is scientific validation for Ibutamoren's role in supporting weight gain, particularly through increased muscle mass, though it remains investigational for this purpose.
