Ephedrine is a sympathomimetic amine derived from the plant Ephedra sinica, historically used in traditional Chinese medicine for respiratory conditions. However, its use for weight loss is grounded primarily in scientific investigation rather than tradition. Clinical studies have demonstrated that ephedrine, particularly when combined with caffeine, modestly increases metabolic rate, promotes thermogenesis, and suppresses appetite, leading to short-term reductions in body weight. A meta-analysis published in JAMA (2003) found that ephedrine and ephedra-containing products produced statistically significant but modest weight loss compared to placebo over several months (mean additional weight loss of 2 kg over placebo).

The proposed mechanism is stimulation of beta-adrenergic receptors, which increases energy expenditure and reduces food intake. However, while evidence supports a short-term effect, the degree of weight loss is moderate, and the risk profile is significant. Adverse events—including hypertension, palpitations, stroke, and even death—prompted regulatory bans in several countries, including the US FDA’s ban on dietary supplements containing ephedra in 2004. Thus, while the evidence base for efficacy exists, it is tempered by significant safety concerns, limiting its use in clinical practice today.
