Ibutamoren (also known as MK-677) is a non-peptide agonist of the ghrelin receptor and stimulates the secretion of growth hormone (GH). Its primary clinical investigation has focused on disorders related to GH deficiency and muscle wasting, rather than directly on weight loss. However, there is some scientific evidence that ibutamoren can influence body composition. Clinical trials in adults and the elderly have demonstrated that ibutamoren increases lean body mass and, in some cases, reduces fat mass or prevents fat gain. The compound increases appetite due to its ghrelin mimetic activity, which may counteract weight loss efforts in some individuals. Nevertheless, by promoting an increase in muscle mass and basal metabolic rate, ibutamoren may indirectly support fat loss or prevent fat accumulation, especially in the context of muscle-wasting conditions or calorie restriction. The evidence is moderate and not robustly conclusive, as most studies are relatively small and of limited duration, and few have focused specifically on weight loss as a primary outcome. There is no established traditional use of ibutamoren for weight loss, and its use in this context is based on mechanistic rationale and preliminary clinical data rather than large-scale, long-term trials.
