The scientific rationale for using papain in wounds and sores is based on its ability to break down proteins, aiding in the removal of necrotic (dead) tissue—a process known as enzymatic debridement. This debridement helps clean the wound, reduce infection risk, and promote the formation of healthy granulation tissue.
Several clinical studies and reviews have evaluated topical papain-based preparations (often combined with urea) for chronic wounds, pressure ulcers, burns, and other skin lesions. Research generally supports their safety and efficacy, especially for wounds with significant slough or eschar. For example, a review in the journal Advances in Skin & Wound Care (2012) notes that papain-urea preparations can be effective and are sometimes preferred to surgical or mechanical debridement due to their gentle action and minimal pain. However, some variability exists in study quality, and allergic reactions have been reported in rare cases. Papain-containing products are regulated in some countries due to concerns over sensitization.
