1. Identity and Botanical Classification

Boerhavia diffusa Linn. is a perennial, creeping herb that belongs to the family Nyctaginaceae (commonly called the four o'clock family). It is native to the tropical and subtropical regions of Asia and surrounding continents. The genus name Boerhavia was conferred in honor of Hermann Boerhaave, a celebrated Dutch physician and botanist of the late 17th and early 18th centuries, while the species epithet diffusa refers to the plant's characteristically spreading, diffuse growth habit.

The plant is most widely known by its Sanskrit name Punarnava — a compound of punar ("again") and nava ("new") — meaning "one that renews" or "one that becomes new again." The plant is also known as Punarnava or Red Spiderling, and is found all over the world, including India, Africa, and the Americas. In India it is additionally referred to by the names Shothaghni, Kathillaka, Varshabhu, Raktapushpa, and Varshaketu. In English-language sources it is sometimes called spreading hogweed.

Punarnava is a perennial, spreading herb with prostrate or semi-upright growth, extensively branched stems, and a thick, tuberous root system. Boerhavia diffusa occurs throughout India. It is also found in Australia, Sudan, Pakistan, Sri Lanka, South Africa, Brazil, and the southern United States. In India, it is an indigenous herb found in the Himalayan region up to an altitude of 2,000 meters in the more temperate parts of the country. Boerhavia diffusa is noteworthy for its flexibility, surviving in a variety of environments ranging from riverbanks to dry terrain.

The overexploitation of this species for use in medicine has led to the decline of the wild population; therefore, community-based cultivation and conservation of the natural population is recommended.

1.1 Plant Parts Used

The therapeutic virtues of the entire plant, from roots to leaves, are used. The roots, in particular, contain a high concentration of bioactive chemicals such as alkaloids, flavonoids, and triterpenoids, which contribute to the herb's medicinal potential. Roots, leaves, aerial parts, and the whole plant have all been employed in different preparations and traditions.

1.2 Common Preparations and Dosage Forms

Boerhavia diffusa is available in multiple preparations. It is available as dried root powder, liquid extract (1:2, hydroalcoholic), capsules (300–500 mg), and herbal tea blends. Classical Ayurvedic formulations prepared from the plant include Punarnavadi Kwath (a water decoction supporting renal function), Punarnavasava (a fermented preparation), and Punarnavarishta (a fermented alcohol-based tonic). Punarnava Mandur is an Ayurvedic formulation combining Punarnava with mandura bhasma (a form of iron calx) and other ingredients, specifically used to treat anemia and liver issues. Punarnavasava is prepared by fermenting fresh herbal extracts for kidney and liver health. In contrast, Punarnavarishta is prepared by the fermentation of a prepared decoction, which is a more potent, alcohol-based tonic for similar conditions.

2. Traditional and Historical Use

Boerhavia diffusa is an important medicinal herb in India's Ayurvedic tradition known as Punarnava. The herb, which thrives in tropical and subtropical areas, has long been prized for its amazing medicinal powers. Its documented use extends across multiple classical Indian medical systems.

2.1 Ayurvedic Tradition

Boerhavia diffusa has a long history of use in various traditional systems of medicine, including Ayurveda, Unani, and Siddha, where it is often regarded as a safe and efficacious herb for the treatment of liver disorders, inflammation, and kidney-related ailments.

Boerhavia diffusa is a plant of the Rasayana category as per Ayurvedic claims. It is reported to possess antiaging, disease prevention, and life-strengthening activities, which hold enormous influence in disease burden and affordability and availability of healthcare in the world. In Ayurvedic medicine, its classification as a Rasayana herb — a category of medicines believed to promote longevity, rejuvenation, and vitality — reflects its status as one of the most broadly valued herbs in the classical tradition.

Historical records from the Charaka Samhita (circa 2nd century CE) mention Punarnava as a key herb for "mutra vishodhana" (urinary purification), praising its "ushna" (warming) potency to invigorate the kidneys. The Sushruta Samhita (4th century CE) recommends Boerhavia diffusa root decoction for mild dropsy and abdominal edema, noting its effectiveness in alleviating water retention without depleting essential salts. Through medieval Ayurvedic texts like the Bhavaprakasha (16th century CE), Punarnava earned a reputation as a "Rasayana" (rejuvenator) that revitalizes the body's fluid balance and strengthens muscle tissue.

The classical Ayurvedic formulation Punarnavadi Kwath has been used specifically to support renal function and urinary tract health. Ayurvedic texts attributed a range of actions to the plant, recorded in Sanskrit as shothaghna (reducing inflammation and swelling), mutravirechana (promoting urination), pandughna (useful in anemia), deepani (improving digestion), and hrudya (beneficial to the heart).

2.2 Preparations Used Traditionally

Ancient practitioners utilized Boerhavia diffusa for a wide range of ailments, most notably for supporting liver and kidney health. The herb was prepared in decoctions, powders, and pastes to aid in the management of jaundice, edema, urinary disorders, and inflammatory conditions. Leaf poultices were applied externally for joint aches and swelling in tribal medicine. In Sri Lanka's Siddha tradition, Punarnava was combined with licorice and ginger as part of an herbal formula to treat urinary calculi.

2.3 Use in Other Traditions and Regions

Revered in traditional medicine in India, China, and Japan, it is primarily used to treat liver ailments, kidney disorders, cardiovascular diseases, and urinary tract infections. It is used in Ayurveda in Asia and Africa as a rejuvenator or "Rasayan" for its excellent antiaging and antioxidant properties. The plant's presence across South America, Africa, and Asia has led to its incorporation into multiple independent folk medicine systems, often for overlapping indications including edema, digestive complaints, and inflammatory conditions.

3. Phytochemistry: Key Constituents and Active Compounds

A systematic review of the literature identified 132 phytochemicals in B. diffusa, including rotenoids, flavonoids, alkaloids, and terpenoids, many of which exhibit antioxidant, anti-inflammatory, and antimicrobial properties.

3.1 Rotenoids (Boeravinones)

The most widely reported rotenoids are the boeravinones A through J. These compounds are a chemically distinct group of isoflavonoid-type rotenoids extracted principally from the roots of the plant. The roots of B. diffusa are the source of a distinct group of isoflavonoids known as rotenoids, along with flavonoids, xanthones, purine nucleosides, lignans, and steroids. Boeravinones have been identified as among the most pharmacologically active isolated fractions of this plant. Boeravinones G and H reportedly inhibited the drug efflux potency of BCRP/ABCG2, the breast cancer resistance protein, a multidrug transporter that induces chemotherapy resistance in cancer cells.

3.2 Alkaloids

The principal alkaloid is punarnavine, which has acquired its name directly from the plant's Hindi designation. Numerous medicinal properties including antioxidant, anti-inflammatory, anti-hyperglycemic, anti-nephrotoxic, cardiotonic, and anti-cancer properties have been found to be present in the plant, and these have been primarily attributed to the presence of the alkaloid punarnavine. A second alkaloid, punarnavine-2, has also been isolated from the roots.

3.3 Phenolic Compounds and Glycosides

The most widely reported phenolics include punarnavoside, trans-caftaric acid, and boerhavic acid. Punarnavoside is a phenolic glycoside found in the roots, and the roots also allegedly contain a phenolic glycoside named punarnavoside. Punarnavoside has been documented as exhibiting antifibrinolytic activity and has been characterized as a potential diuretic agent.

3.4 Flavonoids and Isoflavonoids

The most widely reported flavonoids include borhaavone, quercetin, and kaempferol, along with the isoflavonoid 2′-O-methyl abronisoflavone. These flavonoids contribute substantially to the plant's antioxidant potential.

3.5 Steroids, Lignans, and Other Compounds

Steroids reported include boerhavisterol and β-Ecdysone; lignans include liriodendrin and syringaresinol mono-β-D-glucoside. Various active compounds in B. diffusa include punarnavine, ursolic acid, punarnavoside, liriodendrin, eupalitin, eupalitin-3-O-β-D-galactopyranoside, rotenoids like boeravinones A, B, C, D, E, F and G, quercetin, kaempferol, etc. The roots also contain β-sitosterol, α-2-sitosterol, palmitic acid, hentriacontane, stearic acid, arachidic acid, and ursolic acid.

3.6 Potassium Salts

The herb contains large quantities of potassium salts, which account for its diuretic properties. This mineral composition is directly implicated in the natriuretic and aquaretic effects observed in preclinical research.

4. Mechanisms of Action

4.1 Diuretic Mechanisms

The diuretic activity of Boerhavia diffusa is considered multifactorial. The high potassium salt content of the whole plant provides osmotic support for urinary excretion. These include alkaloids such as punarnavine, flavonoids like quercetin and kaempferol, rotenoids including boeravinones, along with lignans, phenolic compounds, steroids, triterpenoids, glycosides, and saponins. The presence of these compounds contributes significantly to the plant's antioxidant, anti-inflammatory, and organ-protective activities, supporting its traditional use in edema, ascites, and urinary tract disorders.

4.2 Anti-Inflammatory Mechanisms

Punarnavine at a dose of 40 mg/kg b.w. in Balb/c mice for 5 days inhibited the production of proinflammatory cytokines such as TNF-α, IL-1β, and IL-6. It has been shown to stimulate the immune system with enhanced stem cell proliferation, stem cell differentiation, and antibody formation. Ethanol extract of B. diffusa leaves exhibited anti-inflammatory effects with carrageenan-, serotonin-, histamine-, and dextran-induced rat paw edema models.

4.3 Hepatoprotective Mechanisms

The hepatoprotective mechanism is closely linked to the plant's antioxidant capacity. Pretreatment with aqueous and ethanolic extracts decreased the activities of alkaline phosphatase, lactate dehydrogenase, alanine aminotransferase, aspartate aminotransferase, and the level of bilirubin in the serum, which were elevated by acetaminophen. The two extracts also ameliorated the elevation in the activities of the enzymes in the liver. The extracts also protected against acetaminophen-induced lipid peroxidation. These results indicated that leaf extracts from B. diffusa possess hepatoprotective properties against acetaminophen-induced liver damage, which may be mediated through augmentation of antioxidant defenses.

4.4 Immunomodulatory Mechanisms

The alkaloid punarnavine has been the most studied immunomodulatory compound. The alkaloidal fraction of B. diffusa showed immunostimulatory activity in Balb/c mice. This alkaloid fraction possessed "punarnavine," which acquired its name from punarnava, the Hindi name for Boerhavia diffusa.

4.5 Antidiabetic Mechanisms

B. diffusa lowered blood glucose levels via increasing insulin release from pancreatic cells in animal models. A 95% ethanol extract of B. diffusa suppressed inflammatory response both in vivo (mouse gastritis ulcer model) and in vitro (LPS-treated RAW264.7 cells) in a dose-dependent manner (20 to 200 mg/kg b.w.), which is comparable to the standard drug ranitidine (40 mg/kg b.w.).

4.6 Anticancer Mechanisms

One of BD's most well-researched applications is its role in treating cancer. Scientific studies have shown that the plant's compounds can inhibit the growth of cancer cells and prevent angiogenesis, the process by which tumors create new blood vessels to fuel their growth. This anticancer potential, combined with BD's antioxidant properties, positions it as a promising natural therapy in the fight against cancer. These findings, however, are thus far primarily from in vitro and animal studies.

4.7 Cardioprotective Mechanisms

Liriodendrin, isolated from the methanol extract of the roots of B. diffusa, was found to exhibit significant calcium channel antagonistic activity. Scavenging reactive oxygen species (ROS) within the mitochondria may protect the heart against the development of heart failure. Studies with Boerhavia diffusa have demonstrated antioxidant and antihypertrophic potential in H9c2 cardiomyoblast cells. B. diffusa extract contains various bioactive phenolic compounds that are potent antioxidants and plays a significant role in the management of diseases associated with oxidative stress.

5. Scientific Evidence by Area of Use

A critical overview of the evidence base is essential: while traditional use is extensive, scientific evidence primarily consists of in vitro and animal studies, with limited human trials. The following sections describe the state of evidence for each major area, noting where evidence is only preclinical.

5.1 Diuretic and Renal Function

Evidence level: Preclinical (animal); very limited human data.

Boerhavia diffusa is one of the most consistently studied Ayurvedic diuretics. Multiple animal studies have demonstrated significant increases in urine volume and electrolyte excretion following administration of whole-plant or root extracts. The diuretic effect is generally described as natriuretic (promoting sodium excretion) while largely sparing potassium, distinguishing it from some synthetic diuretics.

A safety assessment and nephroprotective evaluation was conducted using acute and sub-acute toxicity studies. The no-observed adverse effect level (NOAEL) of Boerhavia diffusa was 1,000 mg/kg. A study was designed to evaluate the nephroprotective role of Boerhavia diffusa in cisplatin-induced acute kidney injury. Wistar rats (n = 6) were allocated into six groups, administered orally for a period of ten days. On day 10, cisplatin resulted in substantial nephrotoxicity in Wistar rats, with significant (p < 0.001) elevation in serum creatinine and blood urea nitrogen. Boerhavia diffusa at a dose of 200 mg/kg body weight significantly (p < 0.001) ameliorated increased serum creatinine, blood urea nitrogen, oxidative stress, and inflammatory markers. It also exhibited antiapoptotic activity through the reduction of active caspase-3 expression in kidneys. Findings indicate that Boerhavia diffusa is effective in mitigating cisplatin-induced nephrotoxicity.

Classical Ayurvedic texts mention Punarnava as a key urinary herb going back centuries, and these preclinical findings are considered consistent with its traditional role. However, controlled human clinical trials specifically evaluating diuretic efficacy and renal outcomes are absent from the current published literature, and the evidence base must be characterized as preliminary.

5.2 Hepatoprotective Activity

Evidence level: Preclinical (animal and in vitro); no adequate human clinical trials identified.

An alcoholic extract of whole plant Boerhavia diffusa given orally exhibited hepatoprotective activity against experimentally induced carbon tetrachloride hepatotoxicity in rats and mice. The extract also produced an increase in normal bile flow in rats, suggesting a strong choleretic activity.

The results of one study confirmed the hepatoprotective activity of aqueous extracts of B. diffusa at doses of 250 mg/kg and 500 mg/kg against CCl4-induced hepatotoxicity in rats. However, dose adjustments may be necessary to optimize similar hepatoprotective efficacy in clinical settings.

Surange and Pendse evidenced that ethanol extract of root of Boerhavia diffusa had a hepatoprotective effect against country-made liquor-induced liver toxicity in albino rats, with the protective effect evidenced by reduction of elevated alanine aminotransferase (ALT), triglyceride, cholesterol, and total lipids in both serum and tissues, and histopathological studies showed marked reduction in fat deposits in animals receiving Boerhavia diffusa.

Results of a study indicate that Boerhavia diffusa extract possesses potent hepatoprotective activity against CCl4-induced hepatic damage in Swiss albino mice, which is mainly due to its antioxidative properties. Hepatoprotective activity has also been documented against paracetamol (acetaminophen)-induced liver toxicity in rodent models. The consistent cross-study finding is that B. diffusa extracts lower elevated serum liver enzymes (ALT, AST, alkaline phosphatase) and reduce histopathological changes in chemically injured livers. No adequately powered, randomized controlled trials in human subjects have been published to date.

5.3 Anti-Inflammatory Activity

Evidence level: Preclinical (animal and in vitro); mechanistic data available.

The anti-inflammatory activity of Boerhavia diffusa has been studied across multiple preclinical models. Many traditional pharmacological uses of B. diffusa, such as immunomodulatory, anticancer, antidiabetic, antifibrinolytic, anti-inflammatory, diuretic, hepatoprotective, antimicrobial, antifungal, anticonvulsant, and antioxidant activities, have been reported and scientifically validated. The inhibition of proinflammatory cytokines TNF-α, IL-1β, and IL-6 by punarnavine has been reported in mouse models at a dose of 40 mg/kg. Classical carrageenan-induced paw edema models in rodents have consistently shown anti-edematous effects of both aqueous and ethanolic extracts. No controlled human trials have been published.

5.4 Antidiabetic Activity

Evidence level: Preclinical (animal and in vitro); some mechanistic studies; no definitive human trials.

Multiple animal studies have examined B. diffusa's effects on blood glucose. The proposed mechanisms include stimulation of insulin secretion from pancreatic beta cells, inhibition of hepatic gluconeogenic enzymes, free-radical scavenging, and reduction of oxidative stress. The methanol extract showed inhibitory effects on glucosidase, an enzyme involved in carbohydrate digestion, which represents a relevant mechanism for postprandial glucose control. Owing to its numerous bioactivities, including antidiabetic characteristics, in-depth research has been performed on BD. However, controlled trials in humans with diabetes are not represented in the current peer-reviewed literature.

5.5 Anticancer Activity

Evidence level: In vitro and animal studies only; no human clinical evidence.

The anticancer properties of B. diffusa have been explored in multiple cell-line and animal models. As BD reportedly controls unregulated cell growth through S phase inhibition and apoptosis, its cytotoxic activity was effective against many tested cancer cell lines, proving its claimed anticancer potential. An aqueous methanol (3:7) extract of B. diffusa was found to be effective in reducing the metastasis formation by B16F10 melanoma cells. In an anti-cancer evaluation, the methanol extract showed stronger anticancer effects compared with water extract in an in vitro study using the MDA-MB-231 breast cancer cell line. The significance of these preclinical findings for human cancer treatment remains unestablished.

5.6 Immunomodulatory Activity

Evidence level: Animal and in vitro studies; limited human data.

Punarnavine has been the primary focus of immunomodulatory investigations. Studies in animal models have shown both immunostimulant effects (enhanced antibody formation, increased stem cell proliferation) and immunosuppressive effects (inhibition of pro-inflammatory cytokines), depending on the dose and experimental context. These findings align with the plant's Rasayana classification in Ayurveda, which historically implied broader systemic renewal rather than a single directional immune action. No rigorous human immunological trials have been published.

5.7 Cardiovascular and Anti-Hypertensive Activity

Evidence level: Preclinical; limited human data.

Boerhavia diffusa has been attributed with activities relevant to the treatment of conditions such as diabetes, inflammation, stress, hepatotoxicity, jaundice, and heart failure. Some research suggests that an ethanolic extract of Boerhavia diffusa may be able to prevent arsenic trioxide (ATO)-induced cardiotoxicity damage. Liriodendrin's calcium channel antagonistic activity from root extracts is mechanistically suggestive of cardiovascular effects, but clinical trials are absent.

5.8 Antioxidant Activity

Evidence level: In vitro and animal data; well-characterized.

Results of a comprehensive phytochemical analysis revealed the presence of 15 hydroxybenzoic, hydroxycinnamic, acylquinic acids, and their glycosides, one rotenoid, seven flavonoids, 12 fatty acids, and two other glycosides. Among the tested extracts, the methanol extract showed stronger antioxidant ability compared with other extracts. Total phenolic content (TPC) of the B. diffusa extract was estimated to be 123.76 ± 3.43 mg gallic acid equivalents/g extract and total flavonoid content (TFC) was estimated to be 62.51 ± 3.19 mg catechin equivalents/g extract in one laboratory study, indicating a high polyphenol density relative to extract weight.

5.9 Wound Healing

Evidence level: In vitro and animal studies.

A study was aimed to explore the in vitro and in vivo wound healing potential of methanol extract and chloroform extract from B. diffusa leaf and subsequent identification of bioactive metabolites. The study included in vitro cell viability and wound scratch assays as well as in vivo excision wound assays in rat models. Both extracts demonstrated antioxidant properties and promoted wound closure in these experimental systems. Human data are absent.

5.10 Anti-Obesity Activity

Evidence level: Animal and in silico data; no human trials.

An in vivo study was conducted on Sprague–Dawley (SD) rats treated with a high-fat diet to induce obesity, and different parameters such as body weight, behavioral activity, organ fat pad weight, lipid profile, and liver biomarkers (AST, ALT, BUN, and creatinine) were estimated. The result of the study suggested that the phytoconstituents of B. diffusa upon molecular docking revealed the possible binding mechanisms with the cannabinoid receptor and thus show potent anti-obesity action.

6. Body Systems and Health Areas

Various ethnopharmacological reports emphasize Boerhavia diffusa's role in disorders of the reproductive system, gastrointestinal system, respiratory system, urinary system, hepatic system/jaundice, cardiovascular system, and cancer. More specifically:

  • Urinary and renal system: Diuresis, edema reduction, urinary tract infections, urinary calculi, nephroprotection from chemical toxins.
  • Hepatic system: Jaundice, hepatotoxicity (chemical and alcoholic), liver enzyme normalization, choleretic action.
  • Metabolic: Diabetes, hyperglycemia, hyperlipidemia, obesity.
  • Cardiovascular: Cardiac hypertrophy, hypertension, cardiotoxicity protection.
  • Inflammatory and musculoskeletal: Arthritis, joint swelling, edema from inflammation.

The following dosage figures are drawn from study reports and traditional literature as cited in the research reviewed:

  • Animal hepatoprotective studies: Aqueous extracts of B. diffusa at doses of 250 mg/kg and 500 mg/kg against CCl4-induced hepatotoxicity in rats.
  • Animal nephroprotective study: Boerhavia diffusa root extract in doses of 50, 100, and 200 mg/kg administered orally for a period of ten days.
  • Animal anti-inflammatory study (punarnavine): Punarnavine at a dose of 40 mg/kg b.w. in Balb/c mice for 5 days inhibited the production of proinflammatory cytokines TNF-α, IL-1β, and IL-6.
