Etymology

"Chimaphila" comes from the Greek words chima meaning "winter" and philos meaning "loving," a nomenclature that aptly describes the plant's evergreen nature, allowing it to thrive in colder climates and retain its foliage throughout the winter months. "Pipsissewa" derives from the Cree word, which translates to "it breaks into small pieces," a name particularly significant as it reflects the historical and traditional use of the plant in indigenous medicine, where it was commonly utilized for its diuretic properties.

Botanical Description

The plant is native throughout the cool temperate Northern Hemisphere. It grows up to 35 cm (12 in) tall, with one simple stem bearing evergreen, shiny, toothed leaves in opposite pairs or whorls of 3–5 (and sometimes more) along the stem. Leaves have a slightly spiny serrulate margin starting close to the base, ranging from 1½ to 2½ inches long (or longer), with a typically oblanceolate shape. Flowers range from white to pink, produced in a small umbel of 4–8 together. The plant bears stolons, leading to the development of horizontal stems that spread over the ground, producing vertical stems and adventitious roots at the nodes, which along with its dwarf spreading quality make it a decent ground cover.

Geographic Distribution

Pipsissewa has a circumboreal distribution and is found in the boreal forest or taiga in the northern parts of Europe, Asia, and North America. In North America, it is distributed across Alaska, Canada, and throughout many U.S. states including Arizona, California, Colorado, Connecticut, Georgia, Idaho, Illinois, Indiana, Maine, Michigan, Minnesota, Montana, New York, Oregon, Pennsylvania, Virginia, Washington, Wisconsin, and others.

Related Species

Chimaphila umbellata is closely related to Chimaphila maculata. While the two species are similar, and both referred to as Pipsissewa, they are different in appearance. C. maculata has a darker green leaf color with a white stripe down the middle, and its leaves are broader toward the bottom rather than the top.

Common Dosage Forms and Preparations

The leaves of C. umbellata are known to be the most medicinally useful, but all parts of the plant have been used in some remedial way. Preparations found across traditional and more recent herbal literature include:

  • The leaves have been brewed into a tea or used as a flavoring in root beer. A tea can also be made from an infusion of the stems and roots.
  • The leaves of Pipsissewa are used in condiments, candy extracts, flavoring of beer, and tea. Apart from these, the leaves or their extracts are utilized to treat skin sores and boils. Furthermore, stems and roots are used in homeopathic decoctions.
  • The plant is also utilized in perfumery due to its delicate smell produced by methyl salicylate, the essential oil of the plant.
  • Tinctures and fluid extracts were employed in 19th-century Eclectic medicine. The Fluid Extract of Chimaphila (Extractum Chimaphilae Fluidum) was dosed from a half to two drams.

2. Traditional and Historical Use

Indigenous North American Use

Chimaphila umbellata, commonly known as pipsissewa or umbellate wintergreen, has been used by First Nations Peoples of eastern Canada as a traditional medicine for infections, inflammations of various kinds, kidney stones, gonorrhea, stomachache, backache, and coughs; it has also been used as a blood purifier, diuretic, and astringent.

Native American tribes, particularly the Ojibwe, referred to it as "pipsissewa" and employed leaf teas as a diuretic to treat kidney stones and urinary tract infections. Other indigenous groups used it for rheumatism, fever, stomach pains, and backaches, often preparing infusions from the leaves and roots to induce sweating and alleviate inflammatory conditions.

Native Americans such as the Cherokee tribe used the leaves as a tea to help relieve joint pain, colds, fevers, and urinary problems. The Cherokee also used it as a diuretic remedy. They used it in a cloth-like form held over places of skin irritation to treat ringworm, ulcers, and cancer. Native Americans also used the plant to help alleviate snoring, as well as to treat snake bites by swallowing the juice of the leaves. It was also often used for digestive problems, debility, and stimulating appetite.

North American Indians used the plant to increase sweating, to treat fever diseases (like typhoid), and as a fever-reducing agent. They also used it for painful menstruation and tuberculosis of the lymph glands of the neck.

Historically, pipsissewa has been used primarily to treat issues of the blood. It was believed to be a "blood purifier" and as a treatment for coughing up blood, but was also used to help people with kidney and stomach problems, and as an eyedrop for sore eyes.

European Settler Adoption

As European settlers arrived in North America, they encountered pipsissewa and quickly adopted its use into their own herbal practices. During the 19th century, it became a favored alterative, a term used to describe herbs that help to restore balance and promote overall health by supporting the body's natural functions.

Initially, the herb was used by Native Americans to cure sweating and fevers; however, by the 19th century, the early European settlers in North America adopted this plant and used it extensively for treating urinary complaints as well as rheumatic pain. Pipsissewa also gained recognition as a remedy for scrofula, a form of tuberculosis that affects the lymphatic system.

In European folk medicine, the plant was similarly applied for rheumatism and as a tonic for urinary disorders, reflecting its cross-cultural recognition as an astringent and antiseptic remedy.

Eclectic Medicine and Official Pharmacopeial Status

Under its synonym Pyrola umbellata, the dried leaf of Chimaphila umbellata appeared on the United States Pharmacopeia (USP) from 1820 to 1916 and on the National Formulary (NF) from 1916 to 1947. Barton reported its use as an astringent and stimulant, as well as for nephritic affections.

Traditional Preparations

Pipsissewa was typically prepared as a tea or wash, brewed by steeping the leaves in hot water to allow the beneficial compounds to infuse. This tea could be consumed for internal ailments or used externally as a wash to treat skin issues. The versatility of pipsissewa in traditional healing practices underscored its importance in Indigenous North American medicine.


3. Key Constituents and Active Compounds

Overview of Phytochemistry

Chimaphila umbellata has been studied for almost two centuries, with the first paper exploring the phytochemistry of the plant published in 1860. The plant is home to many industrially and medicinally important phytochemicals, the majority of which belong to phenolics, sterols, and triterpenoids. Other important compounds include 5-hydroxymethylfurfural, isohomoarbutin, and methyl salicylate (the only essential oil of the plant). Chimaphilin is the characteristic phytochemical of the plant.

Chimaphila umbellata contains ericolin, arbutin, chimaphilin, and tannin. The plant contains significant amounts of several known biologically active compounds, including arbutin, sitosterol, ursolic acid, and chimaphilin. It has additionally been reported to contain urson and gallic acid.

Chimaphilin

Chimaphilin is a biologically active compound with the IUPAC name 2,7-dimethylnaphthalene-1,4-dione. It is one of the major active phytoconstituents of C. umbellata, extracted from various species of Chimaphila and Pyrola. It is produced in the HGA/MVA pathway (also referred to as the toluhydroquinone or toluquinol pathway) in those plants, though very little has been known about the metabolism of this pathway. Chimaphilin is a member of the 1,4-naphthoquinones and is yellow needle crystal in appearance.

Arbutin (Arbutoside)

Various quinone and phenolic glycosides appear to be important components of this herb, including arbutoside (also called arbutin) and isohomoarbutin, as well as chimaphilin and related compounds. The arbutoside content of pipsissewa leaf is reported to be 75,000 ppm, compared to 270,000 ppm in Arbutus unedo (strawberry tree), 120,000 ppm in Arctostaphylos uva-ursi (uva-ursi), and 90,000 ppm in Vaccinium vitis-idaea (lingonberry).

The medicinal properties of C. umbellata are attributed to key active compounds, primarily arbutin, a hydroquinone glycoside present at approximately 7.5% in the leaves, which hydrolyzes in the urinary tract to hydroquinone, exerting antiseptic effects against bacterial infections. The plant contains about 7.5% arbutin, an antiseptic that is hydrolyzed in gastric fluid to hydroquinone. In alkaline urine, hydroquinone is an effective antimicrobial and contributes to the antiseptic activity in the urinary tract.

Ursolic Acid and Sitosterol

The therapeutic potential of C. umbellata is due to its wide array of phytochemical production including arbutin, sitosterol, ursolic acid, glycoside, hydroquinone, and essential oil. Ursolic acid is a pentacyclic triterpenoid with well-characterized anti-inflammatory and antiproliferative properties documented across multiple plant species, and C. umbellata is recognized as a botanical source of this compound.

Methyl Salicylate

The plant is also utilized in perfumery due to its delicate smell produced by methyl salicylate, the essential oil of the plant. Methyl salicylate is a well-known counter-irritant and analgesic compound, though its contribution to the specific therapeutic actions of Pipsissewa preparations has not been separately quantified in published clinical research.

Ericolin and Tannins

Chimaphila umbellata contains ericolin and tannin in addition to its other constituents. Both Pipsissewa and bearberry (Arctostaphylos uva-ursi) have hydroquinone as a major active ingredient. Pipsissewa has a lower content of tannins than bearberry and more diuretic properties, which may make it more suitable for long-term use.

Hydroquinone exhibits significant antibacterial activity, particularly against common uropathogens, thus aiding in the prevention and treatment of urinary tract infections, which makes pipsissewa a recognized herb in urological herbal applications.

Antifungal Mechanism of Chimaphilin

Bioassay-guided fractionation of Chimaphila umbellata ethanol extracts led to the identification of 2,7-dimethyl-1,4-naphthoquinone (chimaphilin) as the principal antifungal component. Chemical-genetic profiling with a library of yeast deletion mutants indicated that the principal antifungal constituent, chimaphilin, had a complex mode of action, affecting a protein–protein interaction network including 26% of the genes in the profile. A chemical-genetic profile completed with chimaphilin using approximately 4,700 S. cerevisiae gene deletion mutants showed that cellular roles of deleted genes in the most susceptible mutants and secondary assays indicate that the targets for chimaphilin include pathways involved in cell wall biogenesis and transcription.

Antioxidant Activity

Pronounced antioxidant activity of C. umbellata crude extract was identified using the DPPH (2,2-diphenyl-1-picrylhydrazyl) assay, suggesting this phytomedicine has an antioxidant function in wound healing.

Diuretic Properties

Classical Eclectic descriptions characterized Chimaphila as a diuretic, removing dropsical accumulations. The diuretic effect is supported by the plant's hydroquinone glycoside content, though rigorous mechanistic studies in humans are lacking.


5. Scientific Evidence by Area of Use

5.1 Urinary Tract Infections and Urinary Disorders

Evidence characterization: Predominantly traditional/ethnobotanical; very limited clinical evidence; some in vitro and animal data.

Pipsissewa is potentially helpful in any recurrent or chronic urinary tract disorder including incontinence, neurogenic bladder, interstitial cystitis, and radiation cystitis to strengthen bladder function and prevent urinary tract infections. There is, however, a total dearth of clinical trials confirming these traditional indications (and extrapolations from them).

One study reviewed in the literature examined the use of Chimaphila umbellata in a clinical context. Chimaphila umbellata was reported to play a key role in treating urinary tract infections in adolescent girls. A single-blinded randomized study was designed to evaluate the effectiveness of Chimaphila umbellata in adolescent girls aged 13 to 25 years, with 30 cases, most among the 14-year age group. After the intervention, the intensity of clinical manifestations like pain, burning, and itching was reduced. VAS score evaluation concluded that Chimaphila umbellata had a statistically significant effect in treating urinary tract infections in adolescent girls. This study is notable for its use of a randomized controlled design, though it was a homeopathic study with a single-blind design, a small sample (n=30), and did not assess microbiological clearance.

The mechanism proposed for this effect — hydrolysis of arbutin to hydroquinone in alkaline urine — is chemically established but has not been confirmed in well-designed human pharmacokinetic or clinical effectiveness trials specific to C. umbellata. In vitro work with C. umbellata extracts has shown antibacterial effects against Escherichia coli and Staphylococcus aureus, supporting the traditional use for urinary infections, but translation to clinical benefit remains unproven.

5.2 Antifungal Activity (Including Dandruff-Associated Fungi)

Evidence characterization: Preclinical (in vitro and yeast genetic assay); no human clinical trials.

Bioassay-guided fractionation of C. umbellata ethanol extracts led to the identification of chimaphilin as the principal antifungal component. The structure was confirmed by NMR spectroscopy. The antifungal activity of chimaphilin was evaluated using the microdilution method with Saccharomyces cerevisiae (0.05 mg/mL), Malassezia globosa (0.39 mg/mL), and Malassezia restricta (0.55 mg/mL).

Jones et al. (2000), in a survey of potential antifungal phytomedicines used by First Nations Peoples of eastern Canada, reported that extracts of C. umbellata had the greatest antifungal activity of all the materials analyzed. Results with Malassezia strains indicate that chimaphilin has broad-spectrum antifungal activities, including activities against fungal pathogens of humans. The diversity of cellular processes affected by chimaphilin suggests that the compound has multiple targets within the yeast cell, which is expected of an effective antifungal since multiple mutations in diverse pathway genes would be necessary to gain resistance.

Demonstration of antifungal and antioxidant effects supports the traditional use of pipsissewa in treating skin ailments. All available data, however, derive from in vitro laboratory experiments; no human clinical trials for topical antifungal use have been published.

5.3 Anti-inflammatory and Rheumatic Conditions

Evidence characterization: Traditional/ethnobotanical; no controlled human trials identified.

The plant's historical application for rheumatism, joint pain, and inflammatory conditions is extensively documented across multiple indigenous cultures. Indigenous groups used it for rheumatism and inflammatory conditions, often preparing infusions from the leaves and roots to induce sweating and alleviate inflammatory conditions. The anti-inflammatory properties are attributed to ursolic acid and flavonoids such as quercetin and chimaphilin, but these connections have not been demonstrated in controlled human studies specific to C. umbellata. Evidence for the anti-inflammatory activity of ursolic acid exists broadly across multiple plant sources, but has not been confirmed specifically for Pipsissewa in clinical trials.

5.4 Antioxidant Activity

Evidence characterization: In vitro only.

Pronounced antioxidant activity of C. umbellata crude extract was identified using the DPPH assay, suggesting this phytomedicine has an antioxidant function in wound healing. This finding is based on a single standard laboratory assay and has not been validated in human wound healing studies.

5.5 Anticancer / Antiproliferative Activity

Recent studies have examined the antiproliferative effect of C. umbellata against MCF-7 breast cancer cells and its activity against cardiovascular diseases, alongside investigation of the plant against tuberculosis. One study by Fera et al. (2010) suggests that C. umbellata may have anti-cancer properties as it was believed to have in folklore. This work was published as a conference abstract and no peer-reviewed clinical or preclinical study with full methodology has been published. Evidence of anticancer activity should be characterized as speculative and requiring further investigation.

5.6 Hypoglycemic Effects

Chimaphila umbellata has been experimentally shown to have a hypoglycemic effect. The nature and mechanism of this effect have not been described in peer-reviewed human studies, and no clinical trials on glycemic outcomes using this plant have been identified in the published literature.

  • Infusion (tea): The official Infusion of Chimaphila was prepared as ½ ounce (dried leaf) to 16 fluid ounces of water. The dose was 1 to 4 fluid ounces.
  • Fluid extract: The Fluid Extract of Chimaphila was dosed from a half to two drams.
  • Dried leaf (reported traditional dose): One source reports that a traditional dosage for treating acute kidney symptoms is 1–3 grams of dried leaves per cup of water.
  • Homeopathic potencies: The OTC potency range of Chimaphila umbellata in homeopathic form spans 2x–30x, 1c–30c, 200c, 1m, 10m, 50m, and CM. These dilutions contain little to no measurable quantity of active botanical material and are distinct from herbal preparations.

8. Safety Considerations and Known Interactions

Contact Sensitization (Chimaphilin)

Chimaphilin is a yellow naphthoquinone occurring naturally in various Chimaphila and Pyrola species. In Chimaphila umbellata (winter green) and C. maculata, it is a major constituent. Folk medicine recommends the leaves of Chimaphila species as a topical application to treat skin diseases. Since 1887, wintergreen has been claimed to have caused dermatitis and to have been responsible for "idiosyncrasy." Experimental sensitization using the open epicutaneous as well as Freund's complete adjuvant technique has revealed that chimaphilin is a moderate contact sensitizer.

Importantly, though pure chimaphilin is a moderately strong contact sensitizer, topical reactions to whole pipsissewa or crude extracts of it are extremely rare. The fresh leaves, when bruised and applied to the skin, may produce redness, vesication, and desquamation.

Gastrointestinal Adverse Effects

Digestive upset from tannins is the only common problem reported. Taking pipsissewa with food generally avoids this complication.

Arbutin/Hydroquinone Toxicity Considerations

Prolonged or continuous use of products containing arbutin may lead to unforeseen consequences, including potential toxicity or adverse reactions. It is important to limit the duration of use and to monitor for any side effects if Pipsissewa is incorporated into a long-term health regimen.

Pregnancy Contraindication

Pipsissewa is contraindicated in pregnancy. The source of this contraindication is rooted in the historical herbal literature (Tilgner, ND, 1999). No modern clinical safety data in pregnancy are available.

Interaction with Urinary pH

It is advisable to avoid combining Pipsissewa with strongly acidic diets or medications if the primary goal is to achieve urinary antiseptic effects, because the conversion of arbutin to antimicrobially active hydroquinone requires alkaline urinary conditions.

Conservation Status and Sourcing

Pipsissewa is extremely sensitive to trampling. Chimaphila umbellata is "Threatened" in Iowa and Ohio, "Exploitably Vulnerable" in New York, and "Endangered" in Illinois. The plant's alarming conservation status, problems with in vitro cultivation, and research and development issues are recognized challenges for its study and use. This is a slow-growing plant that could easily become endangered. These concerns underscore the importance of using cultivated rather than wild-harvested material wherever possible.

Toxicological Data Gap

Not many toxicological effects have been studied; if not properly dosed and used long-term, it has been suggested that it could be hazardous due to its hydroquinone glycoside content as well as its salicylate content. No formal toxicological dose-response data in humans are available in published literature.
