Sinopsis

Folate deficiency impairs DNA synthesis in erythroid precursors, producing megaloblastic anemia. Folinic acid is an approved treatment for folate-deficiency megaloblastic anemia and is used as 'rescue' therapy in methotrexate-induced bone marrow suppression, restoring folate-dependent thymidine and purine synthesis without requiring DHFR activity.

  • IncontinenciaCientífico

Observational and interventional evidence links folate status to cognitive function in aging, with elevated homocysteine (reflecting folate insufficiency) doubling dementia risk in community cohort studies. Open studies specifically using folinic acid in folate-deficient elderly subjects reported striking improvements in mood and cognition. Folate supports cognitive aging through homocysteine lowering, methylation support, and neuroprotection.

  • Folate, including folinic acid, is implicated in depression because it is required for neurotransmitter synthesis and one-carbon methylation. Patients with depression frequently exhibit functional folate deficiency, low folate is associated with poor antidepressant response, and elevated homocysteine (a marker of folate insufficiency) correlates with depression severity. Folinic acid is used clinically as an adjunct or alternative in patients with impaired folate metabolism.

Folinic acid is the established treatment for cerebral folate deficiency (CFD) syndromes, including FOLR1-mutation–related folate transporter deficiency and folinic acid-responsive seizures (FARS) of the neonatal period. Clinical case series and reports document dramatic seizure reduction and developmental improvement with folinic acid therapy. It is not a general antiepileptic but has specific, well-documented efficacy in folate-transport–related epilepsies.

  • Folinic acid plays a critical role in DNA synthesis, cell division, and methylation reactions required for normal tissue growth. Inborn errors of cerebral folate transport in children cause developmental regression that is reversed or halted by folinic acid therapy. Multiple RCTs have examined high-dose folinic acid in children with ASD, demonstrating improvements in developmental and behavioral outcomes.

  • Folinic acid lowers elevated homocysteine (hyperhomocysteinemia) by donating one-carbon units to remethylate homocysteine to methionine via the one-carbon/folate cycle. Clinical trials in hemodialysis patients show folinic acid reduces plasma homocysteine by approximately 22–44%. Its effect is broadly comparable to equimolar folic acid doses.

  • Folinic acid feeds directly into the folate/one-carbon cycle, supplying the methyl groups carried by 5-MTHF needed to remethylate homocysteine and regenerate SAM (S-adenosylmethionine)—the universal methyl donor for DNA, RNA, protein, lipid, and neurotransmitter methylation. Because it bypasses DHFR, it is particularly useful for supporting methylation in individuals with MTHFR polymorphisms or antifolate drug exposure.

  • Folinic acid is essential for nervous system integrity through its roles in myelin synthesis, neurotransmitter methylation, and neuronal DNA replication. Cerebral folate deficiency—treated with folinic acid—produces leukodystrophy, neuropathy, and neurological regression. Evidence from both inborn-error disease management and observational data in aging populations supports folate's critical role in maintaining nervous system health.

  • ColitisCientífico

Folinic acid supports neurotransmitter synthesis by feeding the folate/one-carbon cycle that generates SAM, the methyl donor required for biosynthesis of serotonin, dopamine, and norepinephrine. Cerebral folate deficiency reduces CSF 5-MTHF, directly impairing neurotransmitter synthesis pathways in the brain. This mechanism underpins folinic acid's investigated roles in depression, ASD, and related neuropsychiatric conditions.

  • ConvulsionesCientífico

Folate, including folinic acid, is essential for periconceptional supplementation to reduce neural tube defect (NTD) risk. The WHO recommends 400 µg/day of folate before and during early pregnancy. High-risk groups (e.g., prior NTD-affected pregnancy) are advised to take 4–5 mg/day. Folinic acid bypasses the DHFR enzyme step, making it a bioavailable alternative to folic acid for supporting fetal development.

  • Folinic acid (leucovorin) is used alongside low-dose methotrexate (MTX) therapy for rheumatoid arthritis to reduce MTX-related toxicity. A Cochrane systematic review found that folate supplementation (folic or folinic acid) reduces abnormal liver function tests and treatment discontinuation, with no clear clinical advantage of folinic acid over folic acid. Folinic acid does not appear to diminish MTX's therapeutic efficacy.
