Deer velvet — more precisely termed velvet antler — is an animal-derived natural product obtained from the rapidly growing, cartilaginous antlers of male members of the family Cervidae before those antlers harden (ossify) into bone. Deer antler velvet (DAV) is harvested from the growing antlers of deer and is a biologically dynamic animal-derived material that has attracted scientific interest due to its regenerative characteristics and complex biochemical composition.

In Traditional Chinese Medicine (TCM) the substance is known under the Chinese name 鹿茸 (pinyin: lùróng), and its pharmaceutical Latin designation is Cornu Cervi Pantotrichum (also written Cornu Cervi Parvum for velvet of young deer). Velvet antler has been used in traditional Chinese medicine (TCM) that classifies many similar substances from a variety of species under the Chinese character name 鹿茸 (pinyin: lùróng) and the commercial name cornu cervi pantorichum.

Source Species

Although there are 45 species of deer in the world, the sika deer (Cervus nippon) and the red deer (Cervus elaphus) have been used more frequently in TCM. Elk (wapiti, Cervus canadensis) is also a commercially significant source. The sika deer is found in East Asia from Vietnam to Taiwan in the south and from China to Korea and Japan in the north, while the red deer originated in Europe and Asia. For the purpose of farming, sika deer were introduced to New Zealand to produce antler velvet, and red deer were introduced to New Zealand, Australia, Chile, and Argentina. The two common species used in the TCM system are sika deer and red deer, which are thought to be useful for treating yang deficiency syndromes.

What Is "Velvet"?

Velvet antler refers to the soft, cartilage-rich stage of deer antlers before they harden into bone. Known for its rapid growth, velvet antler is harvested from species such as sika and red deer during a narrow 60-day window when the antlers are packed with bioactive compounds like collagen, amino acids, growth factors, and essential minerals.

Commercial Forms and Preparations

In Asia, velvet antler is dried and sold as slices, or as a powder which may be boiled in water, usually with other herbs and ingredients, and consumed as a soup. Modern Western supplement forms include encapsulated powder, standardized extracts, tinctures, and sublingual sprays. A 16th-century medical text, Pen Ts'ao Kang Mu, already listed several antler preparations including pills, tinctures, and ointments.

In the traditional commercial trade of Korea and China, whole-stick antler velvet is divided into three sections based upon their supposed properties. In contemporary nutraceutical manufacturing, DAV is processed into water-based extracts, ethanolic extracts, and dried powders. Quantitative measurement of key bioactive ingredients, including collagen, peptides, IGF-1, and polysaccharides, is necessary to ensure consistency across batches. Techniques such as high-performance liquid chromatography (HPLC), enzyme-linked immunosorbent assays (ELISA), mass spectrometry, and amino acid analysis are commonly employed for chemical characterization.

2. Traditional and Historical Use

Traditional Chinese Medicine

The first documented evidence of the use of velvet antler as a health tonic was found on a silk scroll recovered from a Han Tomb in Hunan Province in China. The scroll has been precisely dated at 168 BC and contains a range of significant medical treatments and prescriptions using velvet antler. The first Chinese materia medica, called the Divine Husbandman's Materia Medica or Shennong Bencao Jing, was the first of a series of encyclopedic books that systematically listed medicinal materials or drugs. This text also contains a brief description of deer antler velvet. Its earliest known documentation as a Chinese herb is in the Shen Nong Ben Cao Jing (Divine Husbandman's Classic of Materia Medica); researchers believe that this text is a compilation of oral traditions that were written down sometime between 200 and 250 CE.

In the mid-16th century in China during the Ming Dynasty, deer farming became important in animal husbandry as a means to support TCM. In traditional Chinese medicine, velvet antler has been used for over 2,000 years as a tonic, to improve bone health, to nourish the blood, reduce swelling, and to treat impotence.

Within the TCM framework, Lu Rong is classified according to specific energetic functions. In TCM, deer antler is classified as a potent yang tonic — used to restore warmth, vitality, and energetic balance in the body. Its reputation as a restorative herb is supported not only by traditional texts but also by its historical use. In TCM, Lu Rong is a beneficial herb for Kidney yang and jing deficiencies. Kidney yang and jing deficiency present with symptoms such as fatigue, lassitude, lightheadedness, dizziness, tinnitus, impotence, premature ejaculation, spermatorrhea, nocturnal emissions, soreness or lack of strength of the lower back and knees, cold pain at the lower back and knees, cold extremities, weakness of sinews and joints, and frequent copious, clear urination.

The Chinese traditionally also utilized deer antler velvet as a tonic for children with developmental delays referred to as a "failure to thrive," resulting in slow growth, learning disabilities, and bone problems including rickets. In Chinese medicine, deer velvet has been used to treat impotence, female disorders, urinary problems, skin ailments, and knee weakness.

Korean Traditional Medicine

Koreans use antler velvet to treat anemia and impotence and to stimulate the immune system, improve heart function, muscle tone, lung efficiency, and nerve function. Deer velvet holds a place of significant importance in Korean medicine analogous to its status in TCM, and Korea became one of the world's major importers and producers of farmed deer velvet antler.

Russian and Eastern European Use

Later scientific research on deer antler dates back to the 1980s in Russia. Hundreds of articles since have been published, including those documented by Chinese, Korean, and Japanese scientists. Russian researchers investigated velvet antler as an adaptogenic substance, and Soviet-era sports science explored its potential performance-enhancing properties.

3. Key Constituents and Active Compounds

Overview of Composition

DAV contains proteins, peptides, growth-associated factors, polysaccharides, amino acids, lipids, and minerals, which have been investigated across diverse experimental systems. More specifically, major categories of bioactive components present in DAV include proteins and peptides, growth factors (IGF-1, EGF, TGF-β, FGFs), collagen and glycosaminoglycans, amino acids and minerals (essential and non-essential amino acids, Ca, P, Mg, Zn, Fe), lipids, phospholipids and gangliosides, and polysaccharides and other small bioactive molecules (nucleotides, sialic acid, vitamins).

Proteins, Collagen, and Structural Components

Proteins form a major structural and functional component of DAV, accounting for a significant portion of its dry weight. Collagen, particularly type I and type II, is the most abundant protein and contributes to tissue strength, elasticity, and regenerative capacity. These collagen types are particularly relevant to cartilage and connective tissue physiology. These constituents are particularly important for nutraceutical applications aimed at joint health and anti-aging.

Growth Factors: IGF-1 and Others

Therapeutic effects are attributed to several bioactive compounds, including glycosaminoglycans, uronic acids, sialic acid phospholipids, oestrogens, and notably a range of growth factors such as insulin-like growth factor-1 (IGF-1), nerve growth factor, and epidermal growth factor. Among these, IGF-1 is particularly significant; it is a basic single-chain polypeptide composed of 70 amino acids, known for its powerful role in promoting cell proliferation, stimulating insulin secretion, and enhancing immune function.

Importantly, the concentration and bioavailability of IGF-1 from velvet antler supplements is a subject of active investigation. It is unclear what substances are retained once the velvet is removed from the antler, dried, and formulated as a dietary supplement in the form of capsules, sprays, and tinctures. Research analyzing commercially available products has yielded variable results: no IGF-1 was detected in a Chinese traditional medicine preparation of deer antler velvet tested in one laboratory setting, while the presence of a pharmaceutical protein, human IGF-1, was confirmed in four commercially available products sold as all-natural, nutritional supplements in a separate analytical study, suggesting that some commercial products may be adulterated with synthetic IGF-1.

Glycosaminoglycans and Chondroitin Sulfate

Deer antler contains chondroitin sulfate, a compound that may help treat arthritis. Glycosaminoglycans in general are key structural components of cartilage extracellular matrix, and their presence in DAV has been cited as a plausible mechanism for any observed joint-protective effects. While velvet antler does contain fatty acids, glucosamine, and chondroitin sulfate, it is a relatively expensive ingredient in nutraceutical products and these components are often supplemented through other ingredients.

Lipids, Gangliosides, and Polysaccharides

Though present in smaller quantities, lipids and phospholipids in DAV are biologically significant. They contribute to cell membrane integrity, energy metabolism, and neuroprotection. Gangliosides, which are sialic acid-containing glycosphingolipids, have been reported to support neural development, cognitive function, and anti-inflammatory responses. Polysaccharides in DAV, including both neutral and acidic types, exhibit strong immunomodulatory and antioxidant activities, stimulating immune cell activity and reducing oxidative stress.

Prostaglandins, also present in velvet antler, are known for their anti-inflammatory effects.

Standardization Challenges

Standardization of DAV remains a critical challenge due to the complexity of its composition, variability in antler source, age of the animal, and seasonal factors. These variables mean that the potency and composition of commercially available products can differ substantially from one another and from the materials used in published research, complicating interpretation of study results.

4. Proposed Mechanisms of Action

Anti-inflammatory Pathways

Several in vitro and animal studies have reported that DAV-derived peptides and polysaccharides reduce pro-inflammatory cytokines such as TNF-α, IL-6, and IL-1β, while increasing anti-inflammatory mediators including IL-10. These effects have been observed primarily in macrophage cell lines and rodent models of induced inflammation. These findings remain preclinical; direct translation to human inflammatory conditions has not been established via controlled trials.

Immunomodulation

The constituents of DAV collectively contribute to regenerative, immunomodulatory, antioxidant, and therapeutic properties. Polysaccharide fractions in particular have been identified as likely mediators of immune-stimulating activity in cell and animal models.

Osteogenic and Chondroprotective Effects

DAV contains multiple constituents that may influence bone and cartilage metabolism. Collagen types I and II, chondroitin sulfate, glucosamine, and growth factors such as TGF-β and IGF-1 are all associated with supporting osteoblast activity and cartilage matrix maintenance in experimental systems. In a lumbar facet-joint osteoarthritis–like mouse model, 4-week administration of velvet antler polypeptide extracted from red deer antler led to partial improvement in osteophyte formation and significantly increased articular cartilage formation in the facet joint (P<0.05).

Reproductive and Endocrine Effects

Several studies have suggested that velvet antler polypeptide (VAP) is the main active component in velvet antler, and researchers have tested whether VAP could enhance the level of testosterone in vivo and improve the sexual function of old male mice. These experiments have been conducted in rodent models; robust human clinical data on endocrine mechanisms are absent.

Neuroprotective Effects

Recognizing the potential of velvet antler in the nervous system, as shown in numerous studies, research has been aimed at evaluating the neuroprotective effects of sika deer velvet antler peptide (VAP), along with the underlying mechanisms in neurotoxin-induced Parkinson's disease models. Gangliosides in DAV have been reported to support neural development, cognitive function, and anti-inflammatory responses. This combination of lipids and gangliosides enhances the multifunctional bioactivity of DAV, particularly in brain health and anti-aging interventions. All published neuroprotection data at this time derive from in vitro and animal experiments; no human clinical trials on neurodegenerative outcomes have been published.

5. Scientific Evidence by Area of Use

Overall Evidence Base

Although numerous studies report immunomodulatory, antioxidant, regenerative, neuroprotective, and musculoskeletal-related effects, the strength of evidence varies considerably, with the majority of data derived from in vitro and animal models and comparatively limited well-controlled human trials. The most comprehensive human-level synthesis available is a systematic review published in the New Zealand Medical Journal (2012), indexed in DARE on NIH/NCBI. The aim of this systematic review was to evaluate the evidence from RCTs of velvet antler supplements for any condition, using the QUOROM statement as a guiding framework. (Gilbey A, Perezgonzalez JD. New Zealand Medical Journal 2012; 125(1367): 80–86. PubMed: 23321886.) Seven RCTs were identified as satisfying all inclusion criteria, which examined the effectiveness of velvet antler for rheumatoid arthritis (2), osteoarthritis (1), sexual function (1), and sporting performance enhancement (3). Two RCTs reported some positive effects of velvet antler supplements, but neither were convincing while the remaining five RCTs found no effect. Claims made for velvet antler supplements do not appear to be based upon rigorous research from human trials, although for osteoarthritis the findings may have some promise.

Osteoarthritis

Of all the clinical applications studied, osteoarthritis has the most promising — though still preliminary — human evidence. A study evaluating effects on osteoarthritis (N=53) reported symptomatic relief among participants. In animal research, in a study comparing elk-derived velvet with placebo treatment in dogs with osteoarthritis, improved objective and subjective measures were reported for deer velvet. Preclinical mechanistic data suggest that chondroitin sulfate and collagen peptides from DAV may support cartilage structure, and polypeptide extracts have reduced osteophyte formation in rodent models. The conclusions of the systematic review state that claims made for velvet antler supplements do not appear to be based upon rigorous research from human trials, although for osteoarthritis the findings may have some promise. The evidence for osteoarthritis in humans remains too limited in sample size and methodological rigor to draw definitive conclusions.

Rheumatoid Arthritis

Two RCTs have specifically evaluated elk velvet antler in rheumatoid arthritis. The first was a Phase II trial with dose-finding objectives: forty patients with stage II rheumatoid arthritis were randomly assigned to one of four arms of 10 patients each. One group received placebo and the other three groups received 2, 4, or 6 capsules (215 mg) of elk velvet antler with appropriate placebos to total 6 capsules daily. All subjects continued to take their arthritis medications. At 1 month, there were no significant differences between groups in number of adverse events or health status. The greatest improvement was in the 6 elk velvet antler group, the least was in the placebo group. Differences were not statistically significant. It was concluded that elk velvet antler can be taken safely in conjunction with a number of rheumatoid arthritis medications and should be studied further to assess efficacy.

The larger Phase III trial enrolled significantly more participants: this study examined the effects of elk velvet antler on joint pain and swelling, patient/physician global assessment of disease activity, functional ability, quality of life, blood levels of C-reactive protein, and adverse events in persons with stage 2 to 3 rheumatoid arthritis experiencing residual symptoms after standard treatment. Patients (N=168) were enrolled in a 6-month randomized, triple-blind, placebo-controlled clinical trial. There were no significant differences between groups on any measures. The pattern of change of the measures across time points was essentially the same for both groups. Although some patients reported clinical improvements in their symptoms, there were no statistically significant differences between groups. Overall, elk velvet antler does not effectively manage residual symptoms in patients with rheumatoid arthritis.

In vitro and animal research has provided some mechanistic context: rheumatoid arthritis (RA) is a chronic autoimmune disease that causes joint deformity and disability. Deer velvet antler, a traditional Chinese medicine, has been used to treat various types of arthritis for several thousands of years, but the underlying mechanisms are unknown. Research investigating cellular mechanisms in fibroblast-like synoviocytes and mouse arthritis models has reported anti-inflammatory and anti-arthritic effects at the cellular level, but these findings have not translated into positive outcomes in human trials.

Athletic Performance and Muscle Strength

Three RCTs identified in the 2012 systematic review specifically examined sporting performance. The most detailed available describes 38 active males: to determine the effects of deer antler velvet on maximal aerobic performance and the trainability of muscular strength and endurance, 38 active males were randomly assigned in a double-blind fashion to either deer antler velvet extract (n=12), powder (n=13), or placebo groups (n=13). One trial of 38 males with the treatment of deer antler velvet extract or powder with control showed only isokinetic knee extensor strength and endurance in the powder-taken group were enhanced, but VO2max was not improved and neither was it in the antler extract-taken group. Studies with humans showed different results from animal experiments; the weight of the three groups did not show significant differences, leading to the idea that gained mass by rats may not refer to the increase of muscle but to another different mechanism. The same research found no correlation between antler and erythropoiesis or aerobic performance, as any improvements in the powder or extract group were not observed.

Overall, the evidence for athletic performance enhancement in humans is negative or at best equivocal. Among other claims, deer velvet is said to improve human sport/exercise performance, which has been tested. The available RCTs do not support its use as an ergogenic aid.

Sexual Function

One RCT examined the effect of deer velvet on sexual function in men. Deer velvet is said to improve sexual function in human males, a claim which has been tested and for which there is no evidence. Placebo-controlled, scientifically sound research assessing deer velvet antler's effectiveness as a treatment for enhancing sexual performance and physical strength is absent, although historical, cultural, and folkloric evidence for deer velvet antler's use is present. Preclinical rodent data have investigated testosterone synthesis pathways, but these have not yet been replicated or confirmed in human clinical trials.

Bone Health and Osteoporosis

Both animal and some human data exist in this area. In an osteoporotic rat model, supplementation with a velvet antler-containing preparation significantly increased BMD and bone calcium content, particularly at medium doses, surpassing untreated controls. In a parallel randomized, double-blind, placebo-controlled clinical trial, postmenopausal women who received the capsules for 32 weeks showed marked improvements in lumbar-spine and femoral-neck BMD, bone turnover markers, antioxidant enzyme activities, and reduced low back pain compared to the placebo group. No adverse effects were observed, suggesting that modulation of oxidative stress may underpin the bone-protective effects. This trial used a formulation that combined pilose antler extract with another compound (hydroxytyrosol), making it impossible to attribute effects to velvet antler alone. The bone health evidence overall is preliminary and confounded by multi-ingredient preparations.

Immune Function

Pilose antler/velvet antler, a component of traditional Chinese medicine, is reputed to improve the immune system, physical strength, and sexual function. Experimental evidence for immunomodulation comes primarily from cell culture and rodent studies examining polysaccharide and peptide fractions, which have shown stimulation of immune cell activity. No adequately powered human RCTs specifically measuring immunological endpoints have been published.

Neuroprotection and Cognitive Function

Research in this area is confined to preclinical models. Research was aimed at evaluating the neuroprotective effects of sika deer velvet antler peptide (VAP), along with the underlying mechanisms in neurotoxin-induced Parkinson's disease models. A peptidomic analysis of the VAP, which comprised 189 varieties of peptides, was conducted using LC-MS, with nine sequences identified as significant. These are early-stage findings in animal and cell models; no human clinical data on cognitive or neuroprotective outcomes are currently available.

6. Body Systems and Health Areas Associated with Deer Velvet

  • Musculoskeletal system: Joint health (osteoarthritis, rheumatoid arthritis), bone density, connective tissue repair, and muscle performance — the most studied area, though evidence remains limited.
  • Endocrine and reproductive system: Traditionally used for sexual dysfunction and male reproductive health; animal data suggest interaction with testosterone synthesis pathways; human evidence is negative or absent.
  • Immune system: Immunomodulatory properties observed in cell culture and animal models; no established human evidence.
  • Nervous system: Neuroprotective and cognitive effects investigated in preclinical models involving gangliosides and peptide fractions; no human data.
  • Cardiovascular and hematopoietic system: Traditionally used in Korea to improve heart function and treat anemia; no human clinical evidence for these endpoints.
  • Anti-aging and antioxidant effects: Major biomedical effects of DAV include anti-inflammatory and immunomodulatory actions, antioxidant and anti-aging properties, neuroprotective and cognitive-enhancing effects, osteogenic and cartilage-regenerative potential, wound healing and tissue repair promotion, anti-fatigue and physical performance enhancement, and endocrine and reproductive health support. These multifaceted effects are documented primarily in preclinical research.

7. Dosage Forms and Reported Dosages

Deer velvet is commercially available in multiple forms: encapsulated powders, standardized ethanolic or aqueous extracts, tinctures, and sublingual sprays. Dosages used in published human clinical studies are limited but documented:

  • Phase II rheumatoid arthritis trial (Allen et al., 2002): One group received placebo and the other three groups received 2, 4, or 6 capsules (215 mg) of elk velvet antler with appropriate placebos to total 6 capsules daily. This corresponds to a maximum dose of 1,290 mg/day.
  • Drugs.com clinical data reference: A recent study used 2 to 6 capsules containing 215 mg of deer velvet per day.

No standardized therapeutic dose has been established. Standardization of DAV remains a critical challenge due to the complexity of its composition, variability in antler source, age of the animal, and seasonal factors. The heterogeneity of preparations across studies makes cross-study dose comparison unreliable.

8. Safety Considerations and Notable Interactions

General Safety Profile

The safety surrounding deer velvet as a dietary supplement has not been well researched. The Phase II rheumatoid arthritis trial found that elk velvet antler can be taken safely in conjunction with a number of rheumatoid arthritis medications over the short term, representing one of the few formal human safety assessments.

Chronic Wasting Disease (CWD) and Prion Risk

A documented and peer-reviewed safety concern involves the risk of prion contamination. Chronic wasting disease (CWD) is a contagious, fatal prion disease of deer and elk that continues to emerge in new locations. Research examined prion infectivity in the apical skin layer covering the growing antler (antler velvet) by using CWD-susceptible transgenic mice and protein misfolding cyclic amplification. The finding of prions in antler velvet of CWD-affected elk suggests that this tissue may play a role in disease transmission among cervids. Humans who consume antler velvet as a nutritional supplement are at risk for exposure to prions. This research was published in Emerging Infectious Diseases, a peer-reviewed CDC journal (Angers et al., 2009). Although the most likely means of human exposure to CWD prions is consumption of contaminated venison, the substantial market for velvet in traditional Asian medicine also warrants concern. CWD has been reported in multiple US states, Canadian provinces, and in imported animals in South Korea; the geographic origin and herd health status of source animals is therefore a material safety consideration for any velvet antler product.

IGF-1 Content and Anti-Doping Status

Insulin-Like Growth Factor-1 (IGF-1) is prohibited under S2.4 of the WADA List of Prohibited Substances and Methods. Deer Antler Velvet Spray may contain IGF-1 and WADA recommends therefore that athletes be extremely vigilant with this supplement because it could lead to a positive test. There can be no guarantee that IGF-1 taken orally will not influence the plasma level of IGF-1, which may then influence the result of anti-doping tests.

WADA ruled that the amount of IGF-1 occurring in deer antler velvet does not warrant banishment. However, product adulteration remains a documented concern: the presence of a pharmaceutical protein, human IGF-1, was confirmed in four commercially available products sold as all-natural, nutritional supplements in a published analytical study. Deer antler velvet is not prohibited for US Service Members, but without laboratory testing there is no way to know for certain whether dietary supplements that list deer antler velvet among their ingredients do or do not contain IGF-1, which is on the DoD Prohibited Dietary Supplement Ingredients List.

Regulatory Status

In the United States, deer antler velvet is legally sold as a dietary supplement. It is typically marketed for general wellness support and is not classified as a controlled substance. The U.S. Food and Drug Administration (FDA) permits the sale of deer antler velvet products that comply with labeling and safety standards.

Product Quality and Adulteration

Without laboratory testing there is no way to know for certain whether dietary supplements that list deer antler velvet among their ingredients do or do not contain IGF-1. The analytical literature has documented both products that contained no detectable IGF-1 and products that tested positive for synthetic human IGF-1, indicating substantial variability in the commercially available supply.

Research Gaps and Limitations

Large randomized, placebo-controlled trials evaluating the health benefits of deer velvet antler are needed to assess its therapeutic potential and likely adverse effects. The overall state of the human evidence base is characterized by small sample sizes, methodological heterogeneity, limited trial durations, and the use of non-standardized preparations. Deer antler velvet is marketed for a variety of health benefits, but there is no scientific evidence to support these claims, and its safety as a dietary supplement has not been well studied, according to an assessment by the U.S. Operation Supplement Safety (OPSS) program.
