Verbascum thapsus, the great mullein, greater mullein, or common mullein, is a species of mullein native to Europe, northern Africa, and Asia, and introduced in the Americas, Australia, and New Zealand. The Verbascum genus includes more than 300 species that are predominantly distributed in Asia, Europe, and North America. The plant belongs to the family Scrophulariaceae (the figwort family) and the order Lamiales.

The genus name Verbascum comes from the Latin word barbascum, meaning "bearded," in reference to the hairy stamen filaments of some species. The specific epithet honors the ancient village of Thapsus (now in ruins) located near Carthage in modern-day Tunisia. The common name "mullein" comes from the Latin word mollis, meaning "soft," in reference to the flannel-textured leaves.

Mullein is also known by a variety of common and folk names. It is also known as American mullein, European or orange mullein, candleflower, candlewick, higtaper, and lungwort. "Candlewick plant" refers to the old practice of using the dried down of mullein leaves and stems to make lamp wicks. Some say mullein stems once were dipped in tallow to make torches either used by witches or used to repel them, hence the name "hag taper." "Jacob's staff," "Jupiter's staff," and "Aaron's rod" have all been used as names for the tall flower stalks. The plant's soft leaves are also known commonly as "bunny's ears" and "flannel leaf."

Botanical Description

Common mullein (also known as woolly mullein) is an herbaceous biennial or short-lived perennial with a deep tap root. First-year plants appear as low-growing rosettes of felt-like leaves, with the whorl of leaves emerging from the root crown at the soil surface. Bluish gray-green, oblong to lanceolate leaves are 4–12 inches long and 1–5 inches wide, and are densely covered in hairs.

In the second year of growth, the plant produces an erect, solitary, unbranched, densely hairy flowering stem rising from the center of the rosette to 7 feet tall, topped in summer by a 20-inch long cylindrical inflorescence of densely-packed, 5-lobed, saucer-shaped, sulphur-yellow flowers (each to 1 inch across), with a bloom period from June to September, followed by rounded downy seed capsules (¼ inch diameter) and death of the plant as the seed ripens.

It grows in a wide variety of habitats, but prefers well-lit, disturbed soils, where it can appear soon after the ground receives light, from long-lived seeds that persist in the soil seed bank. It is a common weedy plant that spreads by prolifically producing seeds, and has become invasive in temperate world regions.

Medicinal Parts and Common Preparations

Although it has been used medicinally since ancient times, the popularity of common mullein has been increasing commercially for the past few years. Today, the dried leaves and flowers, swallowable capsules, alcohol extracts, and the flower oil of this plant can easily be found in health stores in the United States.

The top portion of the stem develops yellow flowers, each consisting of a 5-part corolla. These corollas, along with the stamens, constitute the active ingredient. Mullein is a plant called Verbascum thapsus. Dietary supplements that contain mullein are usually made from the flower of the plant. Preparations include dried leaf and flower teas (infusions), fluid extracts, tinctures, encapsulated dried plant material, flower-infused oils (for topical and aural use), and leaf poultices.

The corollas and stamens constitute the active ingredient. The flowers bloom from June to September and have a faint, honey-like odor. The V. densiflorum ("denseflower mullein") species is associated with European mullein and V. thapsus is associated with American mullein products.


2. Traditional and Historical Use

Ancient Mediterranean and European Traditions

The first documented medicinal use of mullein can be attributed to Dioscorides nearly 2,000 years ago, who wrote about its application for pulmonary conditions. Pedanius Dioscorides (40–90 AD) was a Greek botanist, pharmacologist, and physician who was hired as a physician by the Roman Army. He wrote a famous Greek encyclopedia of herbal medicine used for more than 1,500 years. It was Dioscorides who first recommended the use of mullein for pulmonary diseases some 2,000 years ago.

In ancient Rome and Greece, mullein stalks were dipped in tallow and used as torches, giving it the nickname "torch plant." In medieval Europe, mullein was planted around homes as protection from evil spirits and used as a remedy for coughs and colds.

Verbascum thapsus L. (great mullein or common mullein), a medicinal herb indigenous to northern Africa, western and central Asia, and Europe, has been brought to the Americas and Australia. It has been used as a medicine for lung, skin, and throat disorders and has a long history of therapeutic importance, particularly as an astringent and calming agent.

By the 19th century, British Colonial doctors observed mullein's demulcent effects and introduced leaf syrups in Madras Presidency hospitals for "bronchial affections." Modern revivals include the 20th-century German Commission E monographs acknowledging its cough-relieving actions, bridging Western herbalism and Ayurveda.

Native American Traditions

Mullein has appeared in the healing practices of various Native American nations, who traditionally used mullein for respiratory support and as a smoking herb for ceremonial and medicinal purposes. Native to Europe and Asia, it was introduced to North America by early European settlers. Native American tribes recognized its value and began using it for various medicinal purposes, such as treating respiratory issues like coughs and bronchitis.

It was common for Native Americans to use mullein for a wide range of ailments, including abscesses, bruises, sprains, rashes, and burns. Native Americans crushed and sometimes boiled the leaves of mullein, grinding it into a paste that could be massaged onto affected areas.

The Puritans brought mullein seeds to America for their medicinal herb gardens.

Turkish and Central Asian Traditions

Verbascum leaves and flowers are reported to have expectorant, mucolytic, and demulcent properties and are used in traditional Turkish medicine to treat respiratory conditions such as bronchitis, dry coughs, tuberculosis, and asthma. Plants within this genus are also used to treat hemorrhoids, rheumatic pain, superficial fungal infections, wounds, and diarrhea.

Historical Preparation Methods

Traditional uses have focused on the management of respiratory disorders, where mullein was used to treat asthma, coughs, tuberculosis, and related respiratory problems. In its various forms, the plant has been used to treat hemorrhoids, burns, bruises, and gout. Preparations have been ingested, applied topically, and smoked. The yellow flowers were once used as a source of yellow hair dye.

While the whole plant has been used to treat diarrhea and dysentery, and also as an analgesic and antiseptic, the paste obtained from leaves and flowers is used for coughs and pulmonary diseases. Powdered leaves in the form of a poultice are used to relieve joint pain and to soften boils. Dried leaves of V. thapsus are smoked for mental relaxation, whilst tea made from its leaves is used to treat colds and dysentery.

In European herbal medicine, herbalists prepared mullein leaf infusions, teas, and poultices to address a variety of ailments. Mullein's soothing and mucilaginous properties made it a popular remedy for respiratory issues. Herbalists brewed infusions and teas from the leaves to treat coughs, bronchitis, and throat irritations. The anti-inflammatory effects helped reduce inflammation in the airways, while the mucilage provided a protective coating to ease discomfort and facilitate healing. Mullein was also used in steam inhalations to clear nasal congestion and soothe irritated lungs.


3. Key Constituents and Active Compounds

Primary Phytochemical Classes

Constituents of V. thapsus include polysaccharides; iridoid glycosides including harpagoside, harpagide, and aucubin (especially in the leaf); flavonoids, including 3′-methylquercitin, hesperidin, and verbascoside; saponins; and volatile oils.

Plants from the genus Verbascum are a source of a wide variety of chemical constituents, such as iridoids, saponins, flavonoids, phenolic acids, and phenylethanoid glycosides (PhEGs), a class of water-soluble compounds of which the most known is verbascoside. This compound was isolated for the first time in 1950 from Verbascum sinuatum L., from which it got its name, even if it was already known with the name of acteoside.

The activity of molecules such as the flavonoids (apigenin, apigetrin, cynaroside, luteolin, quercetin, kaempferol, rutin, acacetin), iridoids (catalposide, specioside), phenylpropanoids (verbascoside, poliumoside), sesquiterpenes (buddlindeterpene), and saponins (ursolic acid) could serve as inspiration for the design of improved drugs to treat a wide range of pathologies, including respiratory pathologies.

V. thapsus contains a wide variety of phytoconstituents, such as flavonoids; iridoid, phenylethanoid, and phenylpropanoid glycosides; saponins; as well as vitamin C and minerals. The most valuable constituents are coumarin and hesperidin, which possess healing properties.

Verbascoside (Acteoside)

Verbascoside is a polyphenolic compound that belongs to the phenylethanoid glucosides. It occurs in more than 220 plant species. The species with high content of this compound are used in folk medicine and in modern phytotherapy, mostly based on its recognized anti-inflammatory and antimicrobial effects. Studies conducted so far have confirmed these effects, and also pointed to others (i.e., anti-cancer, neuro-, cardio-, hepato-, and nephro-protective).

Verbascoside is hydrophilic and thought to possess pharmacologically beneficial activities, including antioxidant, anti-inflammatory, and antineoplastic properties, in addition to numerous wound-healing properties.

Saponins

Both the leaves and flowers contain mucilage, which is soothing to irritated membranes, and saponins, which make coughs more productive. Saponins, mucilage, and tannins contained in the flowers and leaves may contribute to the soothing topical effects of the plant and its use as an antitussive.

Iridoid Glycosides

Iridoid glycosides such as aucubin have been reported to have anti-inflammatory and antimicrobial effects. The EMA assessment report states that aucubin and verbascoside have anti-inflammatory effects.

Flavonoids

Flavonoids, including quercetin and rutin, are antioxidants that may protect lung tissue from oxidative stress. Most of common mullein's traditional uses could rely on the anti-inflammatory action of phytochemicals, such as quercetin, and this could explain the employment of these plants to treat a wide range of diseases mediated by inflammatory processes such as respiratory diseases, otitis, arthrosis, and rheumatism, among others.

Mucilage

Mucilaginous constituents and thapsic acid are also found in the flowers. The mucilage component forms a soothing, protective layer on irritated mucous membranes, which is the primary physical mechanism underpinning mullein's demulcent properties in the respiratory and gastrointestinal tracts.

Coumarin and Rotenone

The leaves contain rotenone (an insecticide) and coumarin (which prevents blood clotting). The seeds contain several compounds (saponins, glycosides, coumarin, rotenone) that are toxic to fish, and have been widely used as a piscicide for fishing. These compounds are not exclusively concentrated in medicinal leaf and flower preparations, but they are relevant to safety considerations (see Section 7).


4. Established and Proposed Mechanisms of Action

Expectorant and Mucolytic Mechanisms

Verbascum leaves and flowers are reported to have expectorant, mucolytic, and demulcent properties. Saponins are proposed to reduce surface tension within the airways, facilitating the loosening and expulsion of mucus. The mucilaginous polysaccharides coat irritated mucous membranes, reducing friction and soothing inflamed tissue. Together, these properties underpin the traditional antitussive and expectorant uses of the plant.

Anti-Inflammatory Mechanisms

Several lines of evidence demonstrate that phenylethanoids can modulate different molecular pathways underlying inflammatory responses in human cells in vitro and inhibit inflammation in different tissues in vivo. A water-based extract derived from the leaves of V. thapsus has demonstrated significant effects on several anti-inflammatory biomarkers, including IL-6, COX-2, iNOS, and MMPs 1, 8, and 9. Its extract represents a novel treatment option for mild joint inflammation or early stages of osteoarthritis.

In silico results have evidenced affinity between mullein phytochemicals (flavones and O-methylated flavones) and pro-inflammatory cytokines (IL-2 and TNF-α), molecules implicated in inflammatory processes related to the respiratory system. The previously validated anti-inflammatory activity of Verbascum components also supports the potential use of extracts from these plants to achieve the desired anti-inflammatory action.

Antimicrobial Mechanisms

A number of pharmacological activities such as anti-inflammatory, antioxidant, anticancer, antimicrobial, antiviral, antihepatotoxic, and anti-hyperlipidemic activity have been ascribed to this plant. V. thapsus has been shown to be active against bovine herpes virus type 1, and has shown antibacterial and antifungal activity.

Antiviral Mechanisms

Plant extracts from the leaves and flowers of Verbascum thapsus L., which are rich in verbascoside, exhibited antiviral activities against HCoV-229E, HBV, and HSV-2. Such plants have inhibitory activities against murine lymphocytic leukemia and influenza viruses A2 and B.


5. Scientific Evidence by Area of Use

5.1 Respiratory Health

Common mullein (Verbascum thapsus L.) is a medicinal plant readily found in roadsides, meadows, and pasture lands and has been used to treat pulmonary problems, inflammatory diseases, asthma, spasmodic coughs, diarrhea, and migraine headaches.

Animal and in vitro evidence: A 2005 study published in the journal Phytotherapy Research found that mullein extract demonstrated expectorant and bronchodilatory effects in animal models. While animal models do not always translate to humans, this study does lend credence to the traditional use of mullein for respiratory conditions.

Human/clinical evidence: The only clinical study relating to asthma specifically appeared in Dublin, Ireland in the early 1800s and only consisted of seven cases. Current studies show the effectiveness of mullein on other ailments than that of respiratory conditions, necessitating the need for more randomized controlled trials to confirm mullein therapies used in traditional and alternative medicine for lung infirmities.

Mullein leaf has long been recognized in herbal medicine for its lung health benefits, particularly for respiratory relief, anti-inflammatory properties, and antimicrobial activity. While early research suggests promising effects, clinical trials in humans are needed to confirm its efficacy for conditions such as asthma, bronchitis, and viral infections.

Evidence strength: Rich in bioactive compounds such as flavonoids and saponins, mullein is believed to exhibit anti-inflammatory, antimicrobial, and expectorant properties. However, while historical use suggests promising health benefits, clinical research remains limited, and much of the evidence supporting its medicinal applications stems from in vitro (test tube) and animal studies rather than large-scale human trials.

5.2 Ear Infections and Otalgia (Ear Pain)

Mullein (Verbascum thapsus) has a long history of traditional use for earaches and mild ear infections. Traditionally, mullein flowers are infused in oil, often combined with other herbs like garlic or St. John's wort, to create ear drops. Folk medicine from Europe and North America has used mullein oil as a topical remedy for soothing ear pain, especially in children.

Clinical evidence: Various clinical trials conducted on mullein oil show that this herb may work as an herbal remedy for earaches and ear infections. One clinical trial looked at the effectiveness of a natural earache remedy in children. The natural remedy consisted of a variety of herbs, including mullein, garlic, St. John's wort, lavender, and vitamin E in olive oil. The researchers found that the natural mullein oil was effective in reducing ear pain over the course of 2–3 days.

Another clinical trial compared the effectiveness of a natural earache remedy to a standard over-the-counter drug. The natural formula consisted of a blend of mullein and several other herbs, including garlic, calendula, and St. John's wort. The researchers found that the mullein oil earache remedy was as effective as the OTC drug in reducing ear pain.

Scientific evidence supporting mullein's use for ear infections is limited. Some small clinical studies and in vitro research suggest that herbal ear drops containing mullein, in combination with other ingredients, may reduce ear pain.

Evidence strength: The ear pain studies are small, employed multi-herb formulas rather than mullein alone, and did not use placebo controls in all cases. It is therefore not possible to attribute the observed effects solely to mullein. Evidence is preliminary and cannot confirm mullein as an independent effective therapy for otitis media.

5.3 Anti-Inflammatory and Musculoskeletal Applications

Verbascum species (common mullein) have been widely used in Spanish folk medicine to treat pathologies related to the musculature, skeleton, and circulatory, digestive, and respiratory systems, as well as to treat infectious diseases and organ-sense illnesses. These applications support the potential anti-inflammatory action of Verbascum phytochemicals.

In vitro evidence: Osteoarthritis (OA) is a complex disease. OA etiology is complex, multifactorial, and joint-specific. Recently, several studies have suggested a potential adjuvant role for natural extracts on OA progression. In a 2021 study, researchers first analyzed the phenylethanoid glycosides profile and the total amount of polyphenols present in a leaf aqueous extract of Verbascum thapsus L., then investigated the anti-inflammatory and anti-osteoarthritic bioactive potential of the extract in murine monocyte/macrophage-like cells (RAW 264.7) and in human chondrocyte cells (HC), by gene expression analysis of specific inflammatory cytokines, pro-inflammatory enzymes, and metalloproteinases.

V. thapsus leaf extract demonstrated anti-inflammatory and anti-osteoarthritic properties, showing its potential as a therapeutic agent for inflammation and joint conditions.

Evidence strength: Evidence is preclinical (in vitro and animal models only). No human clinical trials on mullein for joint inflammation or osteoarthritis have been published to date.

5.4 Antimicrobial Activity

Research published in the Journal of Ethnopharmacology demonstrated that mullein extracts exhibited antibacterial activity against several respiratory pathogens including Klebsiella pneumoniae and Staphylococcus aureus — bacteria commonly associated with lung infections.

Extracts of V. gypsicola and V. sinuatum have demonstrated antimicrobial activities. The objective of one study was to assess the biological activity of common mullein extracts and commercial products, using selected bioassays for the detection of antibacterial and antitumor activity, and general toxicity. Common mullein has been used for the treatment of inflammatory diseases, asthma, spasmodic coughs, diarrhea, and other pulmonary problems. The study used bench-top bioassays including antibacterial, antitumor, and two toxicity assays — brine shrimp and radish seed. Extracts were prepared in water, ethanol, and methanol.

Evidence strength: Antimicrobial data are derived predominantly from laboratory (in vitro) studies. There are no completed randomized controlled trials in humans specifically testing mullein against bacterial or fungal infections.

5.5 Antiviral Activity

The antiviral activity of verbascoside was also investigated against Dengue virus-2 in Vero and LLCMK2 cells treated with verbascoside for 48 hours, with an EC50 of 3.4 ± 0.4 μg/mL. However, the mechanisms of the antiviral effect have not yet been clarified.

Plants within the Verbascum genus have inhibitory activities against murine lymphocytic leukemia and influenza viruses A2 and B.

Evidence strength: All antiviral evidence is from cell-culture (in vitro) and animal models. No human clinical trials have tested mullein preparations against viral infections.

5.6 Skin and Wound Healing

Mullein has been used as a medicine for lung, skin, and throat disorders. Presently, the dried leaves, flowers, various plant extracts, and flower oil are used in several formulations within Indian traditional medicine.

Mullein leaves were also applied externally to treat wounds, sores, and skin infections. The plant's anti-inflammatory and antimicrobial properties effectively reduced inflammation, prevented infection, and promoted the healing of damaged skin.

Evidence strength: Use for skin conditions and wound healing is documented traditionally across multiple cultures, but clinical trials in humans are absent. Evidence remains at the level of traditional practice and in vitro observations.

5.7 Gastrointestinal Use

Common mullein has been used as an herbal remedy for coughs and diarrhea, and externally against a variety of skin problems. Traditionally used to reduce gut inflammation and support liver function, though scientific validation is lacking.

Evidence strength: Evidence for gastrointestinal use is entirely traditional and ethnobotanical. No clinical studies have specifically examined mullein for gastrointestinal endpoints.

5.8 Anthelmintic Activity

Preliminary phytochemical tests for the powdered materials and extracts of V. thapsus were conducted to determine the presence of flavonoids, saponins, tannins, glycosides, cardiac glycosides, carbohydrates, proteins, and sterols. The anthelmintic activity of V. thapsus was determined using fresh adult roundworms and tapeworms. Test samples of the aqueous methanolic extract (Vt.Cr) were prepared at concentrations of 10, 20, and 40 mg/mL in normal saline. Six worms of approximately equal size of both species were placed in a petri dish containing 25 mL of the test solutions. Solutions of albendazole (10 mg/mL) or distilled water were used as the standard and negative controls, respectively.

Evidence strength: Anthelmintic evidence is preclinical only (in vitro parasite models). Clinical efficacy in human helminth infections has not been demonstrated.


6. Body Systems Associated with Mullein

  • Respiratory system: Primary traditional indication. Has been used to treat pulmonary problems, inflammatory diseases, asthma, and spasmodic coughs.
  • Auditory system: Still prescribed today by naturopathic physicians and medical herbalists as a treatment for chronic otitis media and eczema of the ear.
  • Musculoskeletal system: Verbascum species (common mullein) have been widely used in Spanish folk medicine to treat pathologies related to the musculature and skeleton.
  • Circulatory system: Used in Spanish folk medicine for pathologies related to the circulatory system.
  • Digestive system: Used in Spanish folk medicine for pathologies related to the digestive system.
  • Integumentary system (skin): Used as a medicine for skin disorders, particularly through topical applications of leaf poultices and flower oils.
  • Immune system: The biological effects of verbascoside, such as anti-inflammatory, immunomodulatory, and antioxidant effects, and the underlying mechanisms have been confirmed by in vitro studies.

7. Dosage Forms and Reported Dosages

Preparations of the plant have been ingested, applied topically, and smoked. The following forms and dosages are those reported in source materials and should be understood in the context of traditional use or commercial practice, not as clinically validated therapeutic doses.

Herbal Tea (Infusion)

A typical preparation uses 1–2 teaspoons of cut-and-sifted mullein leaf per 8 oz of hot water. Steep for 10–15 minutes, then strain very well through a fine mesh or cheesecloth to remove tiny hairs.

Ear Oil

Traditionally, mullein flowers are infused in oil, often combined with other herbs like garlic or St. John's wort, to create ear drops. The clinical studies on ear pain used multi-herb formulas in which mullein was a component; the dose of mullein as a single agent was not separately quantified in those trials.

Tincture

Typical tincture dose reported in herbal practice is 1–2 droppers (about 30–60 drops) in water or tea, 1–3 times daily, best used consistently for ongoing respiratory support.

Commercial Supplement Forms

Today, the dried leaves and flowers, swallowable capsules, alcohol extracts, and the flower oil of this plant can easily be found in health stores in the United States. Standardized doses established by human clinical trials do not exist for any mullein preparation for any health indication as of the most recent published literature.


8. Safety Considerations and Known Interactions

General Tolerability

Mullein seems to be well tolerated, but it has not been studied enough to know what side effects it may cause. Mullein is traditionally used for short-to-medium-term support — for example, during a period of respiratory discomfort or seasonal challenges. There is no widely established safety data on continuous, long-term daily use.

Leaf Hairs and Throat Irritation

It is important to thoroughly strain mullein tea to remove the plant's tiny, irritating hairs. These hairs can cause throat irritation if ingested.

Seed Toxicity: Rotenone and Coumarin

Mullein leaf products can be contaminated with the plant's seeds, which contain the toxic compound rotenone. It is crucial to purchase from reputable sources. The plant's leaves, in addition to the seeds, have been reported to contain rotenone, although quantities are unknown.

There are reports of mullein containing coumarin derivatives, which may cause liver toxicity. This adverse effect, however, cannot be confirmed by current scientific research. There are also reports that mullein contains a sapotoxin called rotenone, which is an insecticide, but human scientific evidence is lacking.

Liver and Kidney Considerations

Caution is advised for individuals with severe liver or kidney conditions, especially for prolonged use, due to concerns over coumarin content. Oral use of mullein is not recommended for children due to a lack of safety research and potential toxicity from seed contamination.

Regulatory Status

There is a discrepancy in the literature regarding the FDA's stance on the safety of mullein. As of July 2006, the FDA reported that mullein flowers (Verbascum phlomoides L. or V. thapsiforme Schrad.) are likely safe for use as natural flavoring substances and natural adjuvants in food in small amounts.
